Full-time 40 Hours - Community Health / Primary Care/ Jamaica Plain- Boston, MA ABOUT SOUTHERN JAMAICA PLAIN COMMUNITY HEALTH CENTER Southern Jamaica Plain Health Center (SJPCHC) is a licensed Health Center of Brigham and Women's Hospital and provides multi-disciplinary, comprehensive care including Adult and Pediatric Primary Care, OB/GYN, Cardiology, Neurology, Podiatry, and Behavioral Health. Each year, more than 12,000 patients from Jamaica Plain and surrounding neighborhoods of Boston use these services, producing more than 40,000 visits to SJPCHC. The health center's services are provided by a bilingual English/Spanish, and other languages, medical and professional staff. SJPCHC has a long-standing commitment to comprehensive team-based care. SJPCHC is a teaching site for Harvard Medical School and multiple professionals, universities, and high schools. SJPCHC is widely known for its social justice approach and its extensive programs. These include programs for staff, adults, and youth; and training provided to local, state, and national organizations. SJPCHC’s Health Promotion Center, adjacent to the health center, offers numerous exercise programs, educational groups, and community programs. Together these programs and services are designed to address the health of the whole person, family, and community, addressing the immediate needs of patients, and thinking “upstream” about how to address the social factors and processes – including racism - that impact the health of our patients. The primary responsibility of the Nurse Manager is to coordinate and expedite the clinical operations of a multi-disciplinary team, ensuring an exceptional patient experience. The Nurse Manager reacts to the immediate operational needs of the practice, supervising and directing other members of the interdisciplinary team to further the coordination and advancement of care of patients. Develops strong collaborative relationships across roles, disciplines and departments with a focus that ensures the needs of patients are addressed. The Nurse Manager is accountable for facilitating an appropriate response to the access needs of patients in alignment with practice-based resources.
