About The Position

1st Call Home Healthcare is hiring a full time RN/LPN Homecare Nurse Coordinator to cover our West Michigan Territory. Must love to drive coverage area includes Grand Rapids, Battle Creek and Kalamazoo areas. $$$ 5,000 Sign on Bonus $$$ The Nurse Coordinator assumes responsibility and accountability for the delivery of industry recognized home care nursing and health related services to the agency’s patients

Requirements

• RN/LPN
• Must love to drive coverage area includes Grand Rapids, Battle Creek and Kalamazoo areas

Responsibilities

• Perform start of care visits and establish care plans.
• Monthly follow up visits.
• Perform Supervisory Visits to client and train and supervise caregivers
• Collaborate with interdisciplinary groups and coordinate care
• Handle emergency phone lines on off hours
• Participate in community marketing and customer relation activities
• Perform nurse shift duties
• On occasion, assist Clinical Manager with Quality Assurance chart audits
