About The Position

The Health Care Home Nurse Care Manager is responsible for providing comprehensive care management, care coordination and support for individuals with behavioral and chronic physical health needs. The NCM works closely with clients to bridge gaps in care by integrating both mental health and physical health services, addressing not only clinical concerns but also the social determinants of health that affect the well-being of individuals. This role is especially focused on helping clients navigate the complexities of healthcare systems, overcoming barriers to accessing care, and connecting them with community resources and supports. The Nurse Care Manager's work is essential in promoting health equity, reducing disparities, and improving outcomes for individuals with behavioral health and complex medical needs.

Requirements

  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) with current state licensure.
  • 2 or more years related healthcare experience to include: Inpatient/Outpatient nursing care, medical surgical, psychiatric and pediatric care experience preferred.
  • Knowledge of behavioral health diagnoses, treatment modalities, and medications.
  • Strong communication skills, both written and verbal, with the ability to collaborate effectively with multidisciplinary teams.
  • Experience in care coordination, case management, and patient advocacy.
  • Familiarity with community resources and services for individuals with behavioral health challenges.
  • Ability to work independently and manage a caseload effectively.
  • Healthcare technology software experience required
  • Compassionate, patient, and empathetic approach to client care.
  • Graduate of an accredited school of professional nursing: LPN or higher
  • Basic life support certified

Responsibilities

  • Conduct thorough assessments of clients' physical and behavioral health status to determine adherence to or variance from treatment guidelines and monitoring for treatment gaps utilizing tools for population health management.
  • Develop, implement, and monitor individualized care plans that address both mental and physical health needs.
  • Monitor clients' progress with prescribed treatment plans, including medication management and behavioral interventions.
  • Identify and address potential health issues or barriers to treatment, such as medication side effects or lack of access to care.
  • Collaborate with the interdisciplinary team (therapists, psychiatrists, social workers, etc.) to manage the full array of physical and behavioral health needs, long term community health services and family services and to ensure comprehensive, quality care.
  • Help clients overcome barriers to care, such as lack of transportation, financial constraints, or lack of understanding of treatment options.
  • Act as the primary point of contact for clients, providing guidance and support as needed.
  • Maintain accurate and up-to-date records of client interactions, treatment plans, and progress.
  • Ensure compliance with all regulatory requirements, including HIPAA and other healthcare privacy standards.
  • Completion of point of care labs to screen for metabolic syndrome. Demonstrates accurate interpretation of lab results. Establish and track personalized measurable care goals to ensure effective outcomes.
  • Provide education to clients and families about mental health conditions, medications, lifestyle changes, and coping strategies.
  • Participate in regular team meetings, continued education trainings and case reviews to ensure coordination and continuity of care.
