Registered Nurse (RN) Care Manager – Home Care Charles County MD Flexible Schedule | Autonomy & Support | Immediate Need Lead Care That Truly Changes Lives Options Home Care is seeking an experienced Registered Nurse (RN) Care Manager to support our growing client base and ensure clinical excellence across our services. We are looking for multiple PRN RN Care managers across Washington DC and Maryland. In this role, you’ll bring your clinical expertise, leadership, and compassion directly into clients’ homes—guiding care, supporting caregivers, and ensuring seniors can remain safe and comfortable where they belong. Requirements
- Active Registered Nurse (RN) license in Maryland and/or Washington, DC
- BSN or related healthcare degree — or 2+ years of home care / geriatric nursing experience
- Strong clinical assessment and critical-thinking skills
- CPR/First Aid certification
- Valid driver’s license and reliable transportation
- Professional, compassionate communication with clients, families, and field staff
Responsibilities
- Complete in-home RN assessments
- Develop individualized, person-centered Care Plans in collaboration with clients and families
- Orient caregivers to Care Plans
- Perform supervisory and competency visits
- Support caregivers with clinical guidance and best practices
- Monitor vitals, condition changes, and client status
- Document accurately in our HIPAA-compliant EMR
- Escalate concerns and changes in condition promptly
- Partner closely with Client Care Managers, Care Coordinators, and Scheduling
- Ensure seamless, client-centered care transitions
- Maintain audit-ready care plans and supervisory documentation
- Ensure compliance with state home-care regulations and agency standards
