LPN (KRCC - Breathitt, Lee, Owsley, Wolfe, Letcher, Knott, Leslie, Perry - client's homes) Title: Nurse-1915(i) RISE Company/Location: Kentucky River Community Care/All KRCC Counties/Home visits Credentials: Valid LPN and Board license Employment Type: PRN Special Skills/Knowledge Required: Ability to make home visits; electronic medical record experience; timely completion of documentation; experience with psychotropic medications; excellent communication and interpersonal skills. Primary Functions of job: Medication management for adults with serious mental illness. Provide education and training about medications. Monitoring medication adherence. Timely documentation of services provided. Shift/Days & Work Hours: Varied days and hours based upon client referrals.
