About The Position

Purpose: Assists in the management of the Stroke Program as an expanded role with increased responsibility through the coordination & oversight of stroke program services as a job function expanded role / assignment within the Southeast Michigan (SEMI) Market & works with others to implement & sustain both medical, service line & inter-departmental components of the program. Education & License Requirements: Baccalaureate of Science in Nursing (BSN) degree from an accredited school of nursing required. License as Registered Nurse for State of MI (RN) required. Master of Science in Nursing (MSN) preferred. Stroke related certification preferred Functional Role: Coordinates services to obtain the established goals & metrics for the program; Interacts with patients, colleagues, physicians, clients, members / families & the community in a manner which encourages a safe environment & positive / collaborative relationship; Practice is guided by process & policy / procedure / standards & is consistent with stroke & neuroscience programs within the service line & Trinity Health. Assists in operations, quality of care & program evaluation & development; Develops & maintains program foundations (e.g., protocols, policies & procedures); Supports care practices for all assigned age groups (Child, Adolescent, Adult, Geriatric) as applicable; Provides data capture, analysis & reporting of data to assist leadership team in achieving high quality care, operational efficiency & community health / regulatory alignment; Conducts program audits, produces reports & suggests improvements to processes; Coordinates the development & sustainability of improvement plans; Provides knowledge & expertise in the program, services & applications; Provides support in program accreditation or survey processes & other regulatory agency (e.g., Centers for Medicare & Medicaid Services (CMS)) reviews; Coordinates stroke program certification submission & entry of program requirements & appropriate registry relationships & data, including annual & intracycle surveys; Demonstrates a commitment to safe practices & positive enhancement of patient & colleague experiences; Acts as a resource / liaison / point of contact for others (e.g., patients / families, staff). Requirements

  • Baccalaureate of Science in Nursing (BSN) degree from an accredited school of nursing required.
