Responsibilities include initial and ongoing assessment, individualized recovery planning, and collaborative work with a multi-disciplinary team and discharge planning. Ability to provide person centered individual, group and family interventions in an outpatient recovery oriented program. Ability to develop curriculums and facilitate group experience for clients on multiple topics. Provides case management for assigned clients and collaborative work with outside providers. Participates in ongoing continuing education opportunities particularly focusing on developing skills in person centered planning, Wellness Self-Management, and the FIT modules (co-occurring services). Opportunities to supervise graduate students. Work with and engage a psychiatrically disabled population with multiple problems -- encouraging wellness and recovery. Flexible, problem solving oriented. NYS license required (LCSW, LMSW, LMHC, LCAT).
