About The Position

The Behavioral Health Manager (BHM) will be a critical part of our innovative approach to integrating behavioral health into kidney care clinics. This role specifically focuses on supporting patients undergoing dialysis, who often experience unique mental health challenges, emotional stress, and difficulties adhering to complex treatment regimens. This role will be dynamic and collaborative, requiring agility, openness to innovation, and a passion for pioneering effective patient care strategies. The Behavioral Health Manager is a core member of an integrated care team that includes the patient’s dialysis care team, nephrologists, and social workers, seeing patients in-person and virtually based on-site at the kidney care facility.

Requirements

  • Active independent license in behavioral health in good standing with the appropriate state board (e.g., clinical social work, counseling, psychology) in the state of practice.
  • Master’s degree or higher in a clinical behavioral health field (e.g., social work, counseling, psychology).
  • Minimum of two (2) years of clinical experience working with patients who have co-occurring mental health, chronic medical conditions, substance use, and physical health challenges.
  • Demonstrated experience supporting patients and families facing serious illness, end-of-life concerns, grief, and loss (e.g., long-term care, hospice, palliative care, oncology, or other high-acuity medical settings).
  • Proven ability with brief, structured intervention techniques (e.g., Behavioral Activation, Motivational Interviewing, Problem-Solving Treatment).
  • Demonstrated proficiency in delivering evidence-based psychosocial treatments (e.g., Cognitive Behavioral Therapy, Interpersonal Therapy).
  • Experience conducting biopsychosocial assessments and using standardized screening instruments to support diagnosis and care planning.
  • Clinical comfort and skill in addressing anticipatory grief, bereavement, treatment fatigue, and mortality-related distress within a medical care context.
  • Strong experience partnering with interdisciplinary medical teams, including physicians, nurses, social workers, and psychiatric consultants.
  • Demonstrated ability to translate clinical recommendations into effective team-based care pathways.
  • Comfortable engaging both patients and clinical stakeholders in a busy medical office environment.
  • Demonstrated flexibility and resourcefulness in adapting to workflow changes and evolving clinical processes.
  • Experience working in fast-paced clinical settings with shifting priorities.
  • Capacity to contribute to the development and refinement of clinical workflows and integrated care best practices.
  • Deep commitment to whole-person care, with sensitivity to the challenges faced by patients with chronic conditions.

Responsibilities

  • Conduct biopsychosocial assessments and diagnose mental health and behavioral health conditions using validated screening tools and clinical evaluation.
  • Provide brief, evidence-based therapeutic interventions tailored to kidney care (e.g., Cognitive Behavioral Therapy, Motivational Interviewing, Solution-Focused Brief Therapy).
  • Develop and document individualized care plans aligned with clinical need and patient goals; review care plans collaboratively with nephrology and other care team members.
  • Participate in regular interdisciplinary case conferences, including consultation with a psychiatric consultant, and communicate resulting treatment recommendations to the care team.
  • Maintain accurate, timely documentation of assessments, interventions, treatment plans, progress notes, and recommendations in the electronic care management system to support communication with nephrologists, PCPs, social workers, and other stakeholders.
  • Establish and implement appropriate follow-up schedules; monitor patient response to interventions and adjust care plans in partnership with the care team.
  • Facilitate in-clinic or external referrals to appropriate evidence-based psychosocial services when clinically indicated (e.g., grief support, psychiatric providers, medication-assisted treatment).
  • Utilize psychoeducational materials, brief intervention techniques, and community resources to engage patients, enhance motivation, support behavior change, and promote adherence to care plans.
  • In collaboration with the psychiatric consultant and care team, modify treatment plans for patients not responding to initial interventions.
  • Support patients in developing and sustaining healthy lifestyle behaviors that improve adherence to dialysis and reduce recurrence of challenging symptoms.
  • Collaborate with interdisciplinary stakeholders to refine integrated behavioral health workflows, document best practices, and evaluate outcomes for the kidney care setting.
