You’ll work alongside a passionate team to reduce hospitalizations, build independence, and empower clients to reclaim their lives through recovery-oriented, field-based care. What You’ll Do – Make Every Moment Count Deliver life-changing care by providing substance use counseling and mental health support where it matters most—in the homes and communities of those we serve. Lead with purpose through thoughtful clinical assessments that shape personalized recovery goals and ignite hope. Drive measurable outcomes —stabilize lives, reduce hospitalizations, support housing success, and prevent crises before they escalate. Team up with impact —work side-by-side with a powerhouse ACT team of nurses, employment specialists, peer support, and more to build 360° support for every client. Break barriers to care by helping clients access housing, benefits, medical services, and other critical resources that support independence. Build life skills that last , guiding clients toward long-term recovery, personal growth, and community reintegration. Be a trusted anchor in crisis —step in with clarity and compassion, coordinate care transitions, and support clients through the toughest moments. Tell the story of progress through clear, timely documentation that captures real outcomes, breakthroughs, and next steps in recovery.

Requirements

  • Bachelor’s in social work, psychology, or related field
  • CADC or Substance Certification or in progress + meets MHP criteria
  • Strong communication, critical thinking, and passion for recovery
  • Must pass a background check
  • Valid driver’s license and willingness to work in the field transporting clients in personal vehicle
