About The Position

The Mental Health Case Manager is responsible for providing direct support to clients and their families enrolled in Outpatient MH services, including but not limited to, accessing services, outreach/engagement support, service coordination/referrals, case management, and discharge planning. The Mental Health Case Manager works in conjunction with the MH therapists to assess client and family needs and develop a plan to meet those needs. This position provides clients and their families guidance and assistance in navigating complex systems in order to access services and resources that will support the client in meeting their treatment goals.

Requirements

  • A Bachelor’s degree in Behavioral Health or related field OR a combination of at least three years of education and/or experience in behavioral health field.
  • Care Coordination or Case Management experience at a behavioral health or related agency.
  • Knowledge of state and county systems and resources.
  • QMHA-Registration within 30 days of hire
  • Personal transportation allowing for travel between program sites. If driving a personal vehicle, a valid driver’s license for state of residency, proof of personal auto insurance liability coverage, and a driving record that meets corporate driving criteria is required. If driving a personal vehicle for work purposes more than once a week (4 times per month on average) must have and maintain personal auto liability coverage at $100,000/$100,000 level.
  • Must pass criminal history check

  • Bilingual abilities, including the ability to speak, read, write, and translate documents in Spanish. If Bi-lingual in Spanish, must clear a language proficiency test.

Responsibilities

  • Assesses needs of client and family members, identifying risk factors and creating a case management plan that outlines goals/objectives to meet the client and family needs.
  • Provides service coordination, referral, linkage and advocacy for clients and their families based on assessed needs.
  • Collaborates with caregivers, schools, hospitals, DHS/CPS, MH providers, Wraparound providers, and other entities to ensure client’s needs are met.
  • Provides Client/Family outreach and engagement support.
  • Provides support and guidance to clients and their families during times of transition or crisis.
  • Supports Discharge Planning for clients.
  • Develops and maintains resource list, group therapy options, extracurricular activities, parenting resources, etc.
  • Develops and maintains community and external agency partnerships.
  • Conducts screenings for ancillary services and/or higher levels of care.
  • Provides psycho-education to clients and families.
  • Completes timely clinical documentation for billable and non-billable services in accordance with state, federal and organizational requirements.
  • Maintains efficient and effective clinical and administrative operation practices.
  • Adheres to Outpatient policies, procedures, and protocols and those outlined in the Morrison Child and Family Services Employee Guidebook.
  • Performs other related duties as required and assigned.
  • Demonstrate a commitment to developing a thorough knowledge and application of the Sanctuary model and other organizational policies & practices.
  • Participate in staff development, in-services, and training related to workplace effectiveness; model professional behaviors and implement improvements to business practices with awareness of their impact in a trauma-informed behavioral healthcare organization.
