About The Position

The Medication Safety Officer coordinates the medication safety program across Sanford Health through partnerships with the enterprise Medication Safety Committee, regional Pharmacy and Therapeutics Committees, Regional Medication Safety Committees, the Risk Committee, and the Quality Cabinet. Responsible for the quality and safety of medication administration by all employees at all facilities. Leads in the standardization of the medication safety program and in building a culture of medication safety throughout the enterprise.

Requirements

  • Graduate of an accredited School of Pharmacy.
  • Must have a thorough knowledge of drugs and their actions; knowledge of state and local regulations.
  • License must be in good standing by the State Board of Pharmacy in state(s) of practice.

Nice To Haves

  • Two years of experience preferred.
  • Successful completion of an American Society of Health-System Pharmacists (ASHP) accredited residency is preferred.

Responsibilities

  • Coordinates and standardizes enterprise-wide medication error reporting through an established and accepted error reporting system that utilizes appropriate error detection methods (e.g., trigger tools).
  • Coordinates the development of proactive error prevention strategies.
  • Leads the implementation and evaluation of strategies approved by one or more of partnering committees.
  • Assesses medication use systems in collaboration with pharmacy staff, nursing staff, medical staff, and other healthcare providers.
  • Identifies and evaluates medication system failures.
  • Provides recommendations to partnering committees for opportunities to improve medication use safety.
  • Leads the implementation of medication safety processes approved by one or more of the partnering committees.
  • Collaborates with partnering committees to integrate medication safety into the overall strategic plan for patient safety.
  • Develops methods for spreading best medication-use practices throughout the organization.
  • Uses continuous improvement principles to assess and report the status of efforts to increase medication safety.
  • Leads in the establishment of quality metrics and follow-up monitoring, along with recommended interventions to ensure enhanced safety and effective outcomes associated with safe medication administration practices.
  • The Medication Safety Officer is a visible role and is readily accessible to clinical staff throughout the enterprise.
  • Consistently demonstrates leadership and assumes ownership for the implementation of efficient and effective medication safety practices.
