About The Position

The Medication Care Partner provides basic resident care and assists residents with the self-administration of medication under the direction of the Director of Nursing. This position will be performing duties that ensure resident care remains in accordance with the community’s standards of practice and regulatory requirements at all times. This position promotes or maintains the level of function of residents in their activities of daily living, including but not limited to eating, dressing, grooming, personal hygiene, toileting, and mobility. This position is also responsible for following established departmental policies and procedures, quality assurance program, safety, environmental, and infection control standards. Requirements

  • 3 years of related work experience in resident care
  • High school diploma or equivalent
  • Medication aide training
  • Ability to use a computer
  • Ability to use electronic health record system
  • Proficiency in Microsoft suite
  • Ability to input and retrieve data
  • Ability to speak and comprehend English, the primary language used in the workplace; ability to speak and/or comprehend other languages utilized by residents is a plus.
  • Current BLS or CPR certification
  • Stand on your feet throughout work period. Significant walking or other means of mobility.
  • Possess multi-limb and eye-hand coordination.
  • Able to reach, bend, stoop, push and/or pull, and frequently lift/move up to 50 pounds.
  • Utilize specific vision abilities for viewing work related processes and materials handling.
  • While performing the duties of this Job, the employee is regularly required to stand; walk; use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee is frequently required to sit and taste or smell. The employee is occasionally required to climb or balance and stoop, kneel, crouch, or crawl.
  • Able to sit at a desk, working on computers and operating phones.

Nice To Haves

  • 1 year of supervisory experience is preferred
  • Certified Nursing Assistant (CNA) certificate preferred
  • Ability to speak and comprehend English, the primary language used in the workplace; ability to speak and/or comprehend other languages utilized by residents is a plus.

Responsibilities

  • Assists residents with the self-administration of medications, observes, and documents ordered medications and resident response. Disposes of drugs and narcotics as required and in accordance with pharmacy procedures.
  • Assists residents with activities of daily living, including eating, bathing, dressing, grooming, toileting, transferring and getting to and from activities.
  • Records and reports changes in resident eating habits to the Director of Nursing.
  • Interacts with residents, families, resident representatives, and staff. Answers questions and gives direction and support.
  • Reports changes of condition to the doctor and Director of Nursing.
  • Supervises distribution of meal trays to ensure accurate diets are served and assists residents with meals as needed.
  • Attends the required care plan conferences and other department meetings as needed.
  • Maintains documentation routinely and as needed.
  • Maintains accurate, detailed reports and records.
  • Maintains the confidentiality of all resident data under HIPAA.
  • Plans, assigns, and directs the work of direct care staff during their shift. Determines priorities, makes changes to daily assignments as needed, and explains procedures to be followed.
  • Works closely with the direct care staff and other ancillary staff on their shift to ensure they have appropriate information and supplies to provide proper care to residents.
  • Gives feedback to the supervisor if requested for direct care staff performance.
  • Addresses complaints and resolves problems as they arise, involving the supervisor as appropriate.
