Requirements
- Current Medical Social Worker license in the state of employment (limited licenses not accepted)
- Minimum 1 year of MSW experience in an acute care setting (home care experience preferred)
- Master’s degree in Social Work
- Valid driver’s license and auto insurance
- Ability to meet physical job demands

Responsibilities
- Participate in developing patient care plans
- Conduct patient assessments and identify barriers
- Observe, document, and report patient changes to appropriate staff
- Connect patients and families to community resources
- Assist with financial aid applications and long-term planning
- Counsel patients and families on coping, life changes, and grief
- Complete clinical documentation in accordance with agency protocol and Medicare/Federal guidelines
- Follow all agency policies, procedures, and regulations
- Communicate schedule or availability changes to supervisors
- Attend required trainings and agency meetings
