About The Position

This is a union eligible position. The Integrated Care Center clinic is a federally qualified health center that provides primary care, behavioral health, addiction treatment, psychiatry, dental care, and more. We provide care to those most in need including those who are suffering from homelessness, mental illness, substance use, and chronic mental conditions. We provide team-based care in an integrated care setting. We are looking for a mission-driven service leader who works well in a team, has experience with managing and supporting staff, champions quality improvement, leads change, and is excited to innovate. The On-Call Medical Referrals Coordinator will be responsible for all coordination of care for clinic patients. The (MRC) works to ensure the effective management of all incoming specialist referrals. This position will perform a wide variety of tasks associated with referral tracking to be compliant with Patient Centered Medical Home (PCMH) requirements to ensure quality patient care and safety.

Requirements

  • High school diploma or equivalent accreditation combined with experience working in a medical/clinical setting.
  • Experience working as a patient advocate.

  • Preferably a bachelor's degree in a related field

Responsibilities

  • Ensures that patient's primary care chart is up to date with information on specialty consults, hospitalizations, ER visits, and chronic disease management that is related to their health.
  • Entry of patient clinical and non-clinical information into the assigned electronic registry is the key responsibility of this position (e.g. e-Clinical works, Care Ware, Aries, +Care, etc.).
  • If assigned, maintain ongoing tracking (SFGH alerts) to the Primary care provider team and appropriate documentation on referrals to promote team awareness and ensure patient safety.
  • Effectively assist patients in problem solving potential issues related to the health care system, and financial or social barriers. (e.g., request interpreters as appropriate, transportation services, or prescription assistance).
  • Acts as the system navigator and point of contact for patients and providers.
  • May assume an advocate role on the patient's behalf with the carrier to ensure approval of the necessary services for the patient in a timely fashion.
  • Assists patients with signing up for medical coverage.
