The Medical Record Specialist is responsible for compiling and organizing patient medical records to support patient care and patient safety. Concurrently and retrospectively indexes paper documents into the hospital-wide EMR to ensure accurate, timely, and complete medical records. Processes all discharges for inpatients and outpatients and ensures all records are accounted for and researches any missing medical records. Analyzes individual documents for completion (both physician and non-physician documentation). Validates medical record documentation and works with clinical areas to correct any documentation errors. All activities will be performed in support of the strategy, vision, and values of Phoebe.
