About The Position

POSITION GOAL: The primary purpose/goal of the medical records clerk is to maintain medical records both in the Skilled Nursing and Assisted Living departments in accordance with federal and state guidelines as well as in accordance with our established policies and procedures. Requirements

  • One to two years of related experience and/or training; or equivalent combination of education and experience.
  • Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from groups or managers, residents, customers and the general public.
  • Ability to work with mathematical concepts, such as statistics.
  • Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical solutions.
  • Ability to solve practical problems and interpret a variety of instructions furnished in written or oral form.
  • Proficient personal computer skills including e-mail, record keeping, routine database activity, word processing, spread sheet, and graphics.

Responsibilities

  • Collate new admission packets for Social Service
  • Collate and open new admission chart – placing all needed forms
  • Admissions – door postings, chart prompts, and labeling
  • Maintenance of the contract and agreement file
  • Take new admission picture
  • Admission Welcome cards in room
  • Admission check list – all complete – following 48-72 hours of admission
  • Admission Inventory complete- follow up after 48 hours
  • Discharge – Pull closed charts – review for signatures and all required fields to be completed
  • Mail out satisfaction surveys – track and trend
  • Mail out Care Plan meetings
  • Collect and fax required updates to MCO
  • Collect and fax required information to Peer Review for appeals
  • Complete requested insurance forms
  • Maintenance of all open records
  • Maintenance of closed records
  • Utilization Review - attendance forms and filing
  • Establish a MR sign out and maintain
  • Year end closing of medical records and storage
  • Copy medical records as requested for DPOA, lawyers etc (with permission from HIM)
  • Obtain signature of release of information form
  • Learn coding
  • Audits for accurate door prompts, DNR etc.
  • Responsible for BC Medical Records - Set up system for opening, maintaining, auditing, closing and storing
