Role Scope: This is an opportunity for a mission-driven team player who is passionate about working collaboratively to ensure PACE participants meet their goals. In this role, you will assist with the provision of direct participant care by performing routine care procedures and technical/administrative support services in the clinic. This role is pivotal in providing high quality participant care, including vital signs monitoring, lab testing, phlebotomy, EKGs, immunizations, wound care support, and other procedures as directed by the healthcare provider within your scope of practice. You will partner closely with the interdisciplinary team to share any feedback on the participant’s well-being or changes in status. You will document all specialties in the Electronic Medical Record (EMR). This is a center-based role.

Requirements

  • Passion and mission orientation for serving high-risk seniors and frail older adults.
  • Ability to coordinate and communicate with an interdisciplinary team.
  • Active Medical Assistant license in California and in good standing.
  • Certified in Phlebotomy
  • High School diploma
  • A minimum of two years' experience working with the frail or elderly.
  • Proof of medical clearance for communicable diseases, including a TB test.
  • Proof of all immunizations are up to date.
  • Proof of current CPR/BLS certification required or requirement to obtain within 30 days of employment.
  • Proof of valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law.

Responsibilities

  • Continue to raise the bar. Constructively seek and share feedback and help us implement changes in order to improve clinical outcomes and experience for participants.

  • Performs participant intake, fulfills orders, and manages participant flow in the clinic.
  • Rooms participants in a timely manner, obtaining routine vital signs and screenings.
  • Performs laboratory and testing procedures; follow up with participants regarding test results and treatment plans.
  • Support in back-office duties such as tracking referrals, medical record management / transmission, authorizations, cleaning, and stocking exam rooms.
  • Assists Center Manager in managing inventory, medical equipment, and supplies.
  • Assist with the upkeep of routine clinic logs and clinic equipment readiness.
  • Prepare participants for examinations and treatments, administering immunizations as directed by the healthcare provider.
  • Coordinates with other clinic staff to schedule follow-up visits, external appointments, annual/bi-annual assessments, ensuring a continuity of care.
  • Documents intake in the EMR system.
  • Performs related duties as assigned.
