About The Position

Assess, diagnose, and treat individuals and families with mental health or substance use disorders, including schizophrenia, depression, bipolar, and personality disorders. Coordinate medication management, including bubble packs, injections, and hospital discharge medications. Update and manage a database tracker for hundreds of patients’ injections, Clozaril labs, and annual labs. Conduct triages, refer patients to emergency care or appropriate agencies, and provide home visits per ACT Criteria. Administer injectable medications, lab draws, and other treatments per physician orders. Document assessments and plans in the Next Gen database. Mentor new nurses and medical assistants, ensuring they receive comprehensive onboarding and continuous professional development. Participate in ICC meetings with medical teams to discuss high-risk patients and care coordination. TRAVEL: Position is based out of Mesa, AZ HQ, but travel will be required to visit patients across the greater Metro Phoenix area. RFI Response: Position is based out of Mesa, AZ HQ, but travel will be required to visit about 10 patients per week across the greater Metro Phoenix area.

Requirements

  • Bachelor’s Degree in Nursing, or closely related.
  • 6 months of experience in job offered, Cardiac Progressive Care, Immunization Nurse, or closely related.
  • Assess, plan, implement, and evaluate the psychological well-being of postoperative patients, including identifying and addressing signs of anxiety, agitation, or mood disorders.
  • Assist in the administration of injectable medications while monitoring for any reactions or complications.
  • Provide support to patients and their families about the emotional and psychological aspects of medical treatments and care.
  • Coordinate care between interdisciplinary teams, ensuring communication addresses the psychological needs of patients, particularly those with mood disorders or agitation.
  • Ensure and uphold sterile environments during procedures, including proper sterilization of equipment and adherence to infection control protocols.
  • Provide mentorship and training to new nurses and medical assistants, ensuring they are well-prepared for patient care responsibilities.
  • Must have a valid Registered Nurse License for the state of Arizona.
