Requirements

  • Bachelor's degree and 1 year of relevant experience required
  • Or equivalent combination of education and experience

Nice To Haves

  • Specialized knowledge of Medicaid rules and regulations and New York State Social Service laws preferred

Responsibilities

  • Counsels, guides, advises, and provides outreach services to elderly, chronically ill, and high risk self pay acute care patients for the purpose of securing Medicaid eligibility.
  • Shares Medicaid knowledge by providing consultation to Physicians, Social Workers and other hospital staff who request assistance in obtaining Medicaid coverage for their patients.
  • Provides outreach service to patients referred by other hospital staff via telephone contact and home visitation for discharged SMH patients or SMH inpatients who are unable or unwilling to comply with requirements to secure appropriate documentation for Medicaid Eligibility Examiners, which may also include accompanying or representing the patient at Department of Social Services eligibility interviews, case conferences or fair hearings.
  • Assesses Medicaid referrals to determine patient's potential for Medicaid approval and advises all parties involved of appropriate plan of action.
  • Prepares status reports for department management, including counseling documentation notes for individual patients.
  • Attends weekly meeting with Social Work Discharge Consultation Team to advise team of Medicaid issues and report status of Medicaid cases for SMH patients awaiting discharge with home services or placement in a sub-acute care facility.
  • Maintains good working relationships with professional staff at SMH, Department of Social Services representatives, and staff in various community agencies who can help in the Medicaid process.
  • Exercises judgement to determine if a home visit is a serious safety risk.
  • Keeps co-workers advised of planned visits and takes appropriate safety precautions.
  • Attends Medicaid seminars and reviews New York State Department of Social Services literature to maintain expert knowledge of the Medicaid program.
  • Operates a motor vehicle to transport self and client as necessary.
  • Ensures all safety precautions are taken during operation of vehicle.
  • Other duties as assigned.
