About The Position

Presbyterian Homes & Services is seeking a Registered Nurse to join our RAI/MDS team. This is a full-time, 0.8 FTE position. This position would be primarily day shift hours (flexible if it still meets business needs), completed during a M-F work week (no weekends). No on-call requirement and job duties do not extend outside of MDS role. You will support designated sites in this role, but are able to office at a PHS location convenient to you (provided there is office space available at that location).

Requirements

Registered nurse with current licensure with the State Board of Nursing in state(s) in which he/she practices.
Prior working nursing experience in long term care, transitional or acute care.
Demonstrated compatibility with PHS's mission and operating philosophies.
Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.

Nice To Haves

Previous successful MDS experience preferred.

Responsibilities

The MDS Nurse is responsible for completing, coordinating, implementing and monitoring the comprehensive RAI-Resident Assessment Instrument (MDS) and corresponding assessments, and developing resident care plans consistent with regulations and established clinical best practices.
Working closely with the RAI Director to complete MDS assessments for assigned residents.
Completing the MDS, CAA's (Care Area Assessments), and comprehensive care plans accurately by following the RAI guidelines and within required time frames.
Consulting with interdisciplinary team members for pertinent clinical information.
Utilizing chart audits, resident/remily interviews and communication with direct care staff when completing documentation.
Attending daily IDT phone calls
Ensuring care plans accurately reflect the cares provided.
