About The Position

The purpose of this position is to assess residents’ physical and mental function and document data on minimum data set forms completely and accurately. The MDS Coordinator-RN is also responsible for the documentation of all additional assessments required completely and accurately. The MDS Coordinator-RN shall determine appropriate referrals to other health care professionals and use the Care Area Assessment protocols to determine whether to proceed or not proceed with the overall care plan

Requirements

  • Graduate of an accredited Registered Nurse program.
  • 1 year of experience as a RN with direct experience in a Skilled Nursing Facility required.
  • Ability to add and subtract and perform basic mathematics as it relates to the position is required.
  • Ability to read/write/comprehend the English language in written & verbal form required.
  • Ability to communicate both verbally and non-verbally required.
  • Successfully complete &/or comply with all required background checks to include but not limited to: Ohio BCI&I fingerprint analysis, FBI (if applicable) fingerprint analysis, Nurse Aide Registry, HHS/OIG list of Excluded Individuals/Entities (LEIE) & Ohio Medicaid Provider Exclusion and Suspension List.
  • Valid License to practice as a RN in the State of Ohio.
  • CPR certification required.
  • Able to review and interpret clinical information related to the role of the MDS Coordinator and RN in a Skilled Nursing Facility.
  • Provide services based on physician orders.
  • Communicate physician, patient and family needs with the appropriate caregivers.
  • Intermediate personal computer skills including electronic mail, routine database activity and word processing.
  • Must comply with confidentiality policy.

Nice To Haves

  • Bachelor’s Degree preferred.
  • Previous experience as a MDS Coordinator preferred.

Responsibilities

  • Interview residents, residents’ representatives and others with information related to the minimum data set assessment.
  • Examine residents to collect information related to the minimum data set assessment.
  • Review medical records and records related to residents’ health status to collect information related to the minimum data set assessment.
  • Enter data into the computer program for minimum data set assessments.
  • Print minimum data set assessments are related reports from the computer program and take follow-up action as necessary.
  • Review reports of timely completion of residents’ Care Area Assessments and care plans and take follow-up action as necessary.
  • Participate in the Quality Assessment and Assurance Program, for the nursing department, in cooperation with the Quality Assurance Coordinator.
  • Conduct and/or participate in interdisciplinary team and rehabilitation conference meetings.
  • Complete reports and assignments accurately and adhere to established time schedules.
  • Establish open lines of communication between all nursing personnel.
  • Consistently work cooperatively with administration, nursing service personnel, physicians, community agencies, residents, families and consultants.
  • Act as a licensed nurse when requested and provide care and treatment to residents as requested.
  • Observe infection control procedures.
  • Follow Residents’ Rights policies at all times.
  • Adhere to all facility policies & procedures and ensure they are adhered to by all responsible parties and carry out disciplinary action for non-compliance as appropriate.
  • Ensure adherence to the Corporate Integrity Program and the Performance Expectations at all times.
  • Perform other related duties as directed by the Administrator and/or person of authority.
