We are currently seeking a Full-time qualified facility LPN – MDS Nurse to join our team at Peak Resources Outer Banks! Excellent compensation and benefits with Quarterly bonus potential! Typical Work Schedule is: Monday-Friday, 8:30am-5:00pm, with occasional on-call rotations.

Requirements

Current, unencumbered, active license to practice as an LPN in NC.
Knowledge and experience of the RAI process required
Proficient computer skills
Excellent interpersonal and communication skills
Must not pose a direct threat to the health or safety of other individuals in the workplace
Fulltime MDS Nurse will be required to become RAC certified and maintain recertification every 2 years

Nice To Haves

One to two (1-2) years nursing experience in a hospital or nursing facility
One to two (1-2) years of MDS experience
Knowledge of Medicare and Medicaid processes

Responsibilities

Coordinates and facilitates the timely and accurate completion of both the RAI and Care Management process from admission to discharge in accordance with Company P&P’s, State, and Federal guidelines, and all other entities as appropriate.
The MDS Nurse- LPN will work under the supervision of an RN.
