About The Position

Harbor’s Edge, a premier 5-star, resort-style Continuing Care Retirement Community (CCRC) is located alongside the beautiful Elizabeth River in Norfolk, VA. Our Community consists of 300 Independent Living Residential Units, 33-bed (Medicare/private) Skilled Nursing Healthcare Unit, 40-bed Assisted Living Unit, and 34-bed Memory Support Unit. Additionally, our Community has also been recognized as a “Best” senior living community. We are seeking a highly qualified, full-time MDS Coordinator to join our team to deliver excellent care for our residents. The ideal candidates will have senior living experience (e.g., long-term care, assisted living, memory support) with a passion for working with an elderly population. The MDS Coordinator is responsible for gathering information for the Healthcare community’s current and future residents for assessments, including physical and mental statuses. Reporting to the Director of Nursing, position is responsible to assess resident medical records and communicate with the healthcare team to create applicable health care plans for current and incoming residents. Position is responsible to oversee a variety of assigned programs as well as provide training to healthcare staff regarding facility and regulatory policies and procedures. Essential Personnel Position - Must be available before, during and after inclement weather, emergency, or any disaster situation.

Requirements

  • Current unencumbered Licensed Practical Nurse (LPN) license in state of Virginia.
  • Current Uniform Assessment Instrument (UAI) and Individualized Service Plan (ISP) utilization experience preferred. Will provide training if needed.
  • Current CPR Basic Life Support, AED certification.
  • At least three (3) years of experience in an MDS Coordinator role, or related experience.
  • In-depth knowledge of nursing and medical practices and procedures, laws, regulations and guidelines pertaining to long-term care.
  • Knowledgeable regarding Center for Medicare and Medicaid Services (CMS) regulations, including Assisted Living.
  • Experienced with, and able to successfully manage multiple assigned projects across departments and disciplines.
  • Highly experienced with Quality Assurance, and Compliance programs.
  • Knowledgeable regarding successful training and learning techniques, including multiple training modalities. Accountable for development of comprehensive staff training programs.
  • Extensive experience with Electronic Health Record programs (EMR), with Matrix Care experience strongly preferred.
  • In-depth knowledge of current medical terminology, ICD-10, and CPT codes.
  • Experience with clinical and diagnostic procedures. Excellent technical skills assessment and documentation skills.
  • Knowledgeable regarding various insurance forms and applicable documents related to resident care.
  • Prior experience in a long-term care setting with a strong focus on safety, delivery of high quality healthcare, and appropriate staff education.
  • Excellent leadership, supervisory, and teaching experience.
  • Demonstrate strong attention to detail, organizational and time management skills, high self-initiative, and ability to accurately follow and deliver verbal and written instructions in a timely manner. Utilize excellent listening skills.
  • Skilled at working effectively with the elderly population in a courteous and friendly manner, demonstrating patience and compassion. Perform duties with consideration for residents' rights, demonstrate integrity and discretion in care of residents and their health information.
  • Strong service excellence skills, highly organized, adaptable, time management skills, able to multi-task, and effectively meet deadlines.
  • Consistently demonstrate excellent verbal and written communication skills, communicate in a clear, positive, and engaging manner. Deliver and/or follow verbal and written directives accurately and timely.
  • Knowledgeable with safe and appropriate use of applicable medical supplies, materials, and equipment, within scope of position.
  • Ability and competence to effectively operate computer, applicable software programs, and other standard office equipment. High level of proficiency in a variety of computer programs (e.g., Microsoft Office: Word, Excel, PowerPoint, Relias, EMR, etc.).
  • Perform independently and exercise good judgment.
  • May be required working beyond normal scheduled hours, on weekends, and in other positions as needed.
  • Expected to report to work on time and work the full established schedule.
  • Able to sit, stand for extended periods of time, stationary positions at certain times during the workday, walk, move about the facilities inside and outside, ascend/descend stairs, reach, turn, bend, stoop, crouch, kneel, balance, maintain body equilibrium to prevent falls, reach overhead, grasp with both hands, fine manipulation, coordination, and repetitive motion.
  • Able to exert at least 50 lbs. of energy in order to assist residents in and out of wheelchairs, beds, etc. Must be able to lift up to 25 pounds unassisted.
  • Normal eyesight or use corrective glasses/lenses. Must have three-dimensional vision and ability to observe and identify surroundings, judge distances and spatial relationships so as to see objects where and as they actually are, and to recognize colors.
  • Able to distinguish sounds between normal tones, perceive the nature of sounds, comprehend and exchange ideas and detailed information verbally, as well as engage in activities to convey detailed or spoken instructions to other workers accurately, loudly, and/or quickly.
  • Normal sense of touch and smell.

Nice To Haves

  • American Association of Nurse Assessment Coordinators (AANAC) certification strongly preferred.
  • Current Uniform Assessment Instrument (UAI) and Individualized Service Plan (ISP) utilization experience preferred.
  • Extensive experience with Electronic Health Record programs (EMR), with Matrix Care experience strongly preferred.

Responsibilities

  • Plan, direct and execute compliance and quality assurance audits in Health Services, under the direction of the Director of Nursing.
  • Coordinate efforts of internal and external staff to ensure compliance with federal and state regulations, community policies and procedures, and industry’s best practices.
  • Initiate appropriate programs in a timely manner to ensure survey-readiness.
  • Oversee Medicare and HIPAA Compliance program.
  • Evaluate effectiveness of efforts and recommend adjustments to current practices or programs.
  • Maintain current knowledge of regulations.
  • Complete electronic submission of required documentation to the state database and other entities as required.
  • Complete accurate diagnoses for Patient Driven Payment Model (PDPM) reimbursement.
  • Monitor PDPM scores, looking for potential risks and/or changes that may affect reimbursement.
  • Complete accurate coding of the Minimum Data Set (MDS) with information obtained via medical record review as well as observation and interview with facility staff, residents, and family members.
  • Assures the accuracy of the MDS process for all residents.
  • Maintain current working knowledge of Medicare criteria, servicing as a resource for nursing staff and communicates changes in regulations through continuing education.
  • Oversee Quality Assurance program including risk management assessments and corrective programs.
  • Work closely with the Interdisciplinary Team to assess the needs of the resident and ensures documentation is present in the medical record to support MDS coding.
  • Participate in the Electronic Health Records (EMR) project by sharing expertise on clinical best practices, regulatory requirements, and licensed staff accountabilities.
  • Make reports and recommendations to the Director of Nursing (DON) and Health Services Administrator (HSA) as necessary concerning the operation of the Health Services department.
  • Monitor Medicare assessment schedules for all disciplines.
  • Monitor and maintain care plan schedules.
  • Monitor and update provider schedule for resident certification/recertification.
  • Ensure care plans are kept current with accurate and timely changes and updates in residents’ conditions.
  • Participate in surveys/audits and development/execution of correction plans.
  • Act as resource to other departments in quality assurance and compliance efforts.
  • Collaborate with the team to obtain information to complete end of month triple check process.
  • Assist as needed with providing documentation for medical insurance claims’ disputes.
  • Provide ongoing monitoring and guidance as needed regarding facility quality measures.
