Job Summary and Responsibilities STATEMENT OF PURPOSE: Works under the supervision of the Director of Nursing. The MDS Coordinator/RN is responsible for the coordination of the MDS/Resident Assessment Instrument and assist in the ongoing education of those preparing the MDS. Responsible for the electronic transmission of the MDS/Resident Assessment data to the State Department of Health. Demonstrates the knowledge to complete the MDS/Resident Assessment Instrument. Facilitates communication between the interdisciplinary team. Assist in the assessment of the resident, in the planning of care, and in the evaluation of the plan of care, according to the policies and procedures while maintaining standards of the State Nurse Practice Act. Assists in screening for Skilled Nursing Facility admits. All the criteria-based duties and standards within this document will be performed according to policies, procedures and guidelines of the Medical Center.
