About The Position

As the MDS (Minimum Data Set) Coordinator, you will be responsible for the accurate and timely scheduling, completion, and transmission of MDS assessments. You’ll play a critical role in ensuring compliance with Medicare, OBRA, and Title 22 regulations while supporting care planning and reimbursement processes. This position serves as a liaison between interdisciplinary teams and state and federal regulatory agencies.

Requirements

  • Graduate of an accredited School of Nursing and a current, valid California RN license required.
  • Minimum of one year of nursing experience in a long-term care setting preferred.
  • Strong knowledge of the RAI process, Medicare regulations, and CMS/state documentation guidelines.
  • Proficiency in Microsoft Office applications (Outlook, Word, Excel).
  • Strong interpersonal, organizational, and communication skills.
  • Ability to coordinate multiple priorities, manage deadlines, and maintain accuracy.
  • Ability to sit at a computer and use electronic systems for extended periods.
  • Comfortable communicating with residents, families, and team members across departments.
  • Commitment to confidentiality and professional integrity.

Nice To Haves

  • Minimum of one year of nursing experience in a long-term care setting preferred.

Responsibilities

  • Ensure compliance with all Medicare, OBRA, and Title 22 guidelines.
  • Review and confirm Medicare entitlement and eligibility for prospective residents.
  • Conduct utilization review (UR) and support reimbursement processes.
  • Oversee the collection of data for MDS assessments and care plans.
  • Schedule and facilitate care plan conferences with residents, families, and interdisciplinary team members.
  • Participate in relevant committees and communicate pertinent issues and resolutions.
  • Input or supervise MDS assessment entries, transmit data to the state, and obtain validation reports.
  • Manage the care planning process to ensure timely and appropriate updates.
  • Perform chart audits to confirm documentation supports MDS submissions.
  • Identify residents with significant changes in condition, initiate assessments, and manage interdisciplinary communication.
  • Monitor and ensure MDS documentation and charting support regulatory and billing requirements.
  • Provide timely completion of reports and communicate updates to relevant departments.
  • Perform other related duties as assigned.
