About The Position

Midtown Oaks Post Acute is seeking a detail-oriented and experienced MDS Assistant / Infection Preventionist (MDS/IP Nurse) to join our clinical leadership team in Sacramento. This position supports the MDS Coordinator and Infection Prevention Program, playing a key role in PDPM reimbursement accuracy, regulatory compliance, quality measures, and infection control within our skilled nursing facility. This dual-support role is responsible for: Assisting with MDS 3.0 assessments and care planning Supporting PDPM documentation and reimbursement accuracy Assisting in maintaining the facility’s Infection Prevention & Control Program Ensuring compliance with CMS, CDC, OSHA, and CDPH regulations Promoting strong clinical outcomes and survey readiness

Requirements

  • Licensed Nurse (RN preferred; experienced LVNs considered)
  • Prior experience in MDS or MDS support within a Skilled Nursing Facility (SNF)
  • Knowledge of PDPM, CMS regulations, and care planning
  • Strong organizational, communication, and time management skills

Nice To Haves

  • Infection prevention experience preferred (training available)
  • Experience with PointClickCare (PCC) preferred

Responsibilities

  • Assist the MDS Coordinator with accurate and timely MDS 3.0 assessments
  • Support documentation to ensure proper PDPM reimbursement and case mix optimization
  • Collaborate with the Interdisciplinary Team (IDT) on care plans
  • Help maintain MDS schedules and submission compliance
  • Support audits and ensure strong clinical documentation
  • Monitor quality measures and assist with improvement initiatives
  • Support and help maintain the facility’s Infection Prevention & Control Program
  • Track, trend, and report infections in accordance with regulatory guidelines
  • Assist with infection investigations and required reporting
  • Provide staff education on infection control practices and PPE
  • Help ensure compliance with CDC, CMS, OSHA, and CDPH standards
  • Support prevention strategies to protect residents and staff
