About The Position

The Manager, Strategic Community Partnerships is a critical leader responsible for fostering strong relationships with community partners and ensuring seamless collaboration with care coordination teams. This role focuses on operational excellence, technology adoption, and data-driven strategies to improve patient transitions and partner performance. The Manager will lead market development efforts, oversee platform integration, and implement scalable processes that drive measurable outcomes. Success requires strong analytical skills, clear communication, and the ability to align diverse stakeholders toward shared goals. Location: This position is remote with 15% travel. For consideration you must reside in one of the states we currently do business: AZ, CO, FL, NM, or UT

Requirements

  • Bachelor’s degree in healthcare administration, public health, business, or related field required; advanced degree preferred.
  • Proven track record in healthcare operations, market development, and program scaling.
  • Demonstrated success in managing community partnerships with measurable outcomes.
  • Strong analytical and problem-solving skills, including experience with dashboard management and workflow optimization.
  • Exceptional communication and relationship-building abilities to align diverse stakeholders.
  • Familiarity with healthcare technology platforms and data-driven decision-making.
  • Ability to operate autonomously as a strategic thinker and self-starter.
  • Willingness to travel locally for field support and partner engagement.

Responsibilities

  • Market Development & Strategic Integration - Lead the design and execution of market launch strategies, ensuring care coordination teams and community partners are fully prepared for success. Customize engagement models for each market to meet unique customer needs and drive collaborative outcomes.
  • Community Partnership Leadership - Build and sustain high-impact relationships with community partners to optimize patient transitions and service delivery. Develop and implement engagement frameworks with measurable KPIs (e.g., referral turnaround times, utilization rates). Establish accountability standards for partners and monitor performance against agreed benchmarks.
  • Technology Enablement & Process Optimization - Oversee adoption and performance of care coordination technology platforms, ensuring seamless integration with community partners. Analyze platform functionality and recommend enhancements to improve efficiency and scalability. Leverage data insights to identify workflow gaps and implement automation opportunities.
  • Compliance, Auditing & Quality Assurance - Partner with compliance teams to conduct regular audits of care coordination processes, ensuring adherence to regulatory and quality standards. Drive continuous improvement initiatives to enhance operational excellence.
  • Data Analytics & Performance Management - Maintain and interpret referral dashboards to uncover trends, gaps, and opportunities. Translate analytics into actionable strategies that improve partner engagement and team performance.
  • Field Support & Workforce Development - Provide hands-on support in the field to promote billable service delivery and strengthen partner relationships. Lead onboarding and training programs for new hires, including post-training evaluations and ongoing development initiatives. Champion best practices to ensure efficiency and scalability across markets.
  • Leadership & Strategic Communication - Serve as a key liaison between internal teams and external partners, aligning stakeholders toward shared goals. Communicate strategic priorities clearly and persuasively to drive organizational alignment. Collaborate with senior leadership to deliver on growth objectives and operational targets.
