About The Position

The Manager of Healthy Living Programs is responsible for leading and operationalizing initiatives that promote healthy living and improved health outcomes for both patients and the broader community. This role oversees key programs, including but not limited to, the Food as Health and Exercise is Medicine initiatives, while identifying opportunities to expand and integrate healthy living strategies across the Community Health Network. The Manager serves as a community collaborator, strategic planner, and relationship manager, fostering partnerships and aligning efforts with community organizations, clinical teams, and system leaders. Reporting to the Community Health Network in support of the Community Benefit Corporation, this role is accountable for driving program implementation, streamlining key processes, and ensuring initiatives are both impactful and sustainable. This position typically reports to Director, Community Benefits Initiatives or similar role within the Community Health Network.

Requirements

  • Bachelor's degree in Healthcare Administration, Public Health, Social Work, Nursing, Business or related field required
  • Minimum of five (5) years of experience with proven track record in program development with ability to design, implement and scale community health programs.
  • Three (3) years of direct management experience in not-for-profit community work, preferably in health care with experience in building and maintaining collaborations with nonprofits, public health agencies, food banks, and/or community organizations.
  • Strong communication (oral and written) and presentation skills to represent programs at community events, leadership meetings, or public forums.
  • Ability to work collaboratively with health care professionals and community partners at all levels to achieve established goals and improve outcomes.
  • Understanding of health disparities, non-medical drivers of health (NMDOH), and health barriers affecting underserved populations.
  • Knowledge of concepts associated with performance management, business planning, and targeted outcomes.
  • Proven track record of problem solving skills.
  • Proficiency in MS Office Suite (Word, Powerpoint, Excel).

Responsibilities

  • Provides leadership and strategic direction for healthy living programs, e.g. including the Food as Health initiative, system food pantries, Exercise is Medicine and aligned community fitness efforts, and other non-medical drivers of health (NMDOH) aligned wellness programs.
  • Develops and implements standard operating procedures for food pantry management, nutrition programs, and fitness initiatives, ensuring efficient, safe, and high-quality service delivery.
  • Integrates nutrition, food access, and physical activity programs into patient care pathways and community health strategies, addressing key non-medical drivers of health. This includes identifying emerging trends, technologies, and best practice in nutrition, fitness programming, and community health to enhance the quality and impact of programs.
  • Establish and maintains partnerships with food banks, community organizations, and public health agencies to expand program reach and effectiveness.
  • Defines and monitors key performance indicators (KPIs), tracks program outcomes, and uses data insights to continuously improve program design, impact, workflows and reporting for leadership and stakeholders. This includes oversight of program budgets and resource allocation.
  • Works closely with clinical team and community health workers to align food, fitness, and initiatives focused on addressing NMDOH with broader care coordination efforts.
  • Promotes community education on healthy eating, nutrition literacy, the importance of physical fitness, and healthy living principles through outreach events and partnerships.
  • Builds and maintains partnerships with community organizations, Memorial Hermann health care teams, and local stakeholders to expand the reach and sustainability of programs.
  • Responsible for effective communication to senior leaders and collaborative partners through personal contact, presentations, and reporting.
  • Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
  • Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
