About The Position

Become a part of our caring community and help us put health first The Manager, Care Coaching is responsible for the oversight of evaluation of member's needs and requirements to achieve and/or maintain optimal wellness state. The Manager, Care Coaching works within specific guidelines and procedures; applies advanced technical knowledge to solve moderately complex problems; receives assignments in the form of objectives and determines approach, resources, schedules and goals. The Manager, Care Coaching oversees the evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and well-being of members. Decisions are typically related to resources, approach, and tactical operations for projects and initiatives involving own departmental area. Requires cross departmental collaboration, and conducts briefings and area meetings; maintains frequent contact with other managers across the department. Use your skills to make an impact

Requirements

  • Bachelor's Degree in a health related field
  • 2 or more years of managed care experience in Medicare and/or Medicaid.
  • 1 or more years of management/leadership experience.
  • Proficiency in analyzing and interpreting trends
  • Comprehensive knowledge of Microsoft Word, Excel and Access
  • Progressive experience in the health solutions industry, with emphasis with leading and managing teams
  • Must be passionate about contributing to an organization focused on continuously improving consumer experiences
  • This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
  • This role is part of Humana's Driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits
  • Must reside in the State of Indiana

Nice To Haves

  • Master's Degree in human services, social sciences, or health related field.
  • Applicable State licensure in field of study
  • RN or LPN with current State Licensure
  • Proficiency in analyzing and interpreting financial trends
  • Previous experience in utilization management, discharge planning and/or home health or rehabilitation
  • Experience with health promotion, coaching and wellness
  • Health Plan experience
  • Previous Medicare/Medicaid Experience
