The Program Manager of Adult Residential Programs is responsible for the supervision, management and oversight of all Jefferson Center adult residential facilities and acting Administrator to Inn Between and Karen’s House and for supervision/management of Alternative Care Facilities (ACF) and Mental Health Transitional Living (MHTL), Case Management/Nursing Home Case Management Teams. Direct supervision of Coordinator of Nursing Home/ACF Team and Residential Counselors. Direct supervision of Transportation; direct oversight of certification paperwork (27-65) for Office of Behavioral Health.

Requirements

Master's Degree in counseling, psychology or related field, plus one to two years related experience and/or training
Minimum of 1 year of experience supervising the delivery of personal care services that include activities of daily living
An active, unrestricted Colorado nursing home administrator license
An active, unrestricted Colorado registered nurse license plus at least six months of work experience in health care during the previous ten year period
An active, unrestricted Colorado licensed practical nurse license plus at least one year of work experience in health care during the previous ten (10)-year period
A bachelor’s degree with emphasis in health care or human services plus at least one year of work experience in health care
An associate’s degree with emphasis in health care or human services plus at least two (2) years of work experience in health care during the previous ten (10)-year period
Thirty (30) credit hours from an accredited college or university with an emphasis in health care or human services plus three (3) years of work experience in health care during the previous ten (10)-year period
Five (5) or more years of management or supervisory work in the field of geriatrics, human services, or providing care for the physically and/or cognitively disabled during the previous ten (10)-year period
A college degree in any field plus two (2) years of health care experience during the previous ten (10)-year period
Licensure required (LCSW, LPC, LMFT, or LP) required.
Computer skills required (Excel; Word; email; etc.)

Responsibilities

Clinical, programmatic and administrative duties for all residential programs
Direct supervision of Residential Leads staff, Coordinator and Case Managers of ACF/Nursing Home Team
Acts as assigned “Administrator” and primary liaison with the Department of Health for residences overseen, including completing, maintaining and submitting all reports and records required by the Department
Responsible for ensuring alignment and compliance with all Department of Health Guidelines and standards, including (but not limited to): annual licensing requirements, flu/covid reporting guidelines, emergency preparedness, sufficient training of all new and existing staff, overseeing comprehensive resident admissions and discharges, ensuring resident rights are prioritized, food safety, protection and proper storage of health information records, addressing physical plant standards, and proper waste disposal.
Acts as “QMAP Supervisor” and evaluates competency of all newly hired QMAP staff and performs regularly scheduled evaluations based on CDPHE standards
Oversees internal Quality Management Program to track patterns, identify training gaps, and ensure resident satisfaction
Participate in annual (and other) Department of Health inspections and audits; respond to audits as needed
Hiring, training, supervision and performance evaluations of staff
Coordinate utilization of contract/pool staff for coverage
Time sheet approval
Review, track and respond to referrals for placement and coordinate with referral sources/agencies and consumer’s insurance provider including conducting assessments for appropriate placement at the residential facilities
Facilitate transitions of residential consumers into and out of the program
Fiscal oversight, including annual budget requests and oversight of expenses
24 hour availability to residential staff (on rotation with other residential staff and supervisors) for emergency situations
Development and maintenance of policy & procedure manual for facilities
Any and all other duties as currently required and that may become required in the future per 6 CCR 1-1011-1 Chapter 7 guidelines pertaining to the Code of Colorado Regulations for Assisted Living Residences
Direct supervision of Transportation Lead position and oversight of Transportation Department
Annual reporting as requested for all programs (BHA, Health Dept., FRHP, etc.)
Facilitate team meetings
Complete annual performance evaluations of assigned staff
Hire new staff, train new and existing staff
Ensure Department of Health regulations are met at each facility, including cleanliness and safety. Participate in Department of Health inspection and audits. Respond to audits with written plans as indicated
Monthly oversight and review of 27-65 caseload, documentation and participate in annual BHA Site Review of certified charts audited
24 hour availability to residential staff for emergency situations, shared with the designated residential staff, including on call rotation.
Participation in Barriers to Discharge meeting with State Hospital/Fort Logan
Coordinate referrals for all programs, maintain census/caseloads, communicate any changes to appropriate clinical/support staff
Track and monitor data/outcomes for programs and report to Executive Management as scheduled
Attend mandatory in-services, compliance with individualized training plan if required
Participate in supervision by coming prepared with agenda. Report high/risk problem cases, and utilize a problem solving approach as well as feedback
Attend supervision on times and intervals agreed upon with supervisor
Exhibit enthusiasm, courtesy, adaptability, flexibility, and spirit of cooperation in the work environment
Maintain effective interpersonal relations with consumers, peers, subordinates, upper management, visitors and the general public
Effectively respond to client/consumer needs and problems, initiate and maintain positive interactions, timely response to phone calls, email and other requests
Ensure staff adheres to Center performance standards; utilize data as defined by Executive Management Staff
Participate in assigned work groups/committees including team meetings, Intensive Network Managers meetings, and Nursing Home/ACF meetings/trainings
Preparation of annual budget for residential programs/services
Annual training of Nursing Home Staff per and State of Colorado BHA standards
Annual training for community ACF staff as indicated/appropriate
Regularly scheduled meetings of Family Care Providers
Assist Jefferson Center Development Department in grant applications and reporting as requested
Maintain communication and links with other Jefferson Center programs/departments, e.g. Access, Inpatient, Outpatient, Marketing, etc.
