Requirements
- Current California LVN license
- Experience in home health, hospice, or in-home care preferred
- Experience caring for adults with physical disabilities
- Must have experience providing catheter care, including proper maintenance, monitoring, and infection prevention in a clinical or home health setting
- Strong communication, critical thinking, and problem-solving skills
- Compassionate, dependable, and patient-focused
- Basic Life Support (BLS) certification required
- Ability to work flexible hours, including evenings and weekends
- Reliable transportation and valid driver’s license
Nice To Haves
- Experience in home health, hospice, or in-home care preferred
Responsibilities
- Perform and manage catheter care, including maintenance, monitoring, and infection prevention
- Provide skilled nursing care in patients’ homes across pediatric, adult, and geriatric populations
- Administer medications, treatments, and therapies as prescribed by the physician
- Monitor and assess patient health status, vital signs, and clinical progress
- Collaborate with patients, families, and interdisciplinary care teams to develop and implement individualized care plans
- Educate patients and families on disease processes, medication management, catheter care, and home care techniques
- Respond promptly and professionally to changes in patient condition and emergency situations
- Maintain accurate, timely documentation in electronic medical records (EMR)
- Participate in ongoing training and professional development to maintain and enhance clinical competency
