Requirements

  • Current California LVN license
  • Experience in home health, hospice, or in-home care preferred
  • Experience caring for adults with physical disabilities
  • Must have experience providing catheter care, including proper maintenance, monitoring, and infection prevention in a clinical or home health setting
  • Strong communication, critical thinking, and problem-solving skills
  • Compassionate, dependable, and patient-focused
  • Basic Life Support (BLS) certification required
  • Ability to work flexible hours, including evenings and weekends
  • Reliable transportation and valid driver’s license

Nice To Haves

  • Experience in home health, hospice, or in-home care preferred

Responsibilities

  • Perform and manage catheter care, including maintenance, monitoring, and infection prevention
  • Provide skilled nursing care in patients’ homes across pediatric, adult, and geriatric populations
  • Administer medications, treatments, and therapies as prescribed by the physician
  • Monitor and assess patient health status, vital signs, and clinical progress
  • Collaborate with patients, families, and interdisciplinary care teams to develop and implement individualized care plans
  • Educate patients and families on disease processes, medication management, catheter care, and home care techniques
  • Respond promptly and professionally to changes in patient condition and emergency situations
  • Maintain accurate, timely documentation in electronic medical records (EMR)
  • Participate in ongoing training and professional development to maintain and enhance clinical competency
