About The Position

La Casa Mental Health Rehabilitation Center (MHRC) is a 190-bed program providing assistance to adults 18 and older. Telecare's mission is to deliver and manage excellent services and systems of care for persons with serious mental illness. The LVN/LPT Supervisor provides supervision to Mental Health Workers in their assigned area, including hiring, evaluating, scheduling, assigning duties, and overall coordination of client/resident care.

Requirements

  • Two (2) years of experience in medical nursing in a skilled nursing facility or acute care setting.
  • Graduation from a Vocational Nursing program.
  • Current licensure (LVN/LPT).
  • Valid CPR Certificate.
  • Understanding of psychiatric illness and treatment.
  • Knowledge of psychotropic medications.
  • Understanding of behavior management.
  • Knowledge of the laws and regulations governing the delivery of health services in the State of California.
  • Applicant must receive clearance from the Department of Justice.

Responsibilities

  • Demonstrate the Telecare mission, purpose, values, and beliefs in everyday language and contact with clients, the public, and other staff members.
  • Hire, evaluate, and take appropriate corrective action for Mental Health Workers under her/his direction.
  • Plan, organize, and direct services provided to clients/residents.
  • Utilize management practices that include empowerment of staff, provision of clear and concise expectations, and frequent feedback consistent with Continuous Quality Improvement.
  • Promote and maintain harmonious and effective relationships and communications within the department and other departments.
  • Regularly inform the supervisor of client/residents’ behavior and condition.
  • Notify the supervisor of residents who present a clinical management problem.
  • Provide safe, effective, and efficient implementation of direct care in accordance with established policies, procedures, and standards of care.
  • Establish and maintain a therapeutic relationship with clients and act as a role model in maintaining a hopeful and positive attitude in problem-solving and coping with disabilities associated with mental illness.
  • Participate in the admissions of new clients.
  • Participate in the discharges of clients.
  • Understand and integrate into practice a clear expectation of the Recovery Model of service delivery.
  • Perform therapeutic psychiatric and medical tasks.
  • Administer, document medication, and provide prescribed treatments to clients.
  • Complete assigned documentation tasks accurately and thoroughly, including transcribing physician orders appropriately and in a timely manner.
  • Participate in the development, review, and updating of care plans.
  • Review the MHW’s documentation and ADL sheets daily.
  • Assure that the monthly recaps of physician’s orders, medications, and treatment records are completed.
  • Provide one-to-one interventions with clients.
  • Practice universal precautions at all times.
  • Participate as a team member and provide input via reporting observations, concerns, and asking appropriate questions.
  • Review and implement client treatment plans.
  • Actively participate in multidisciplinary team meetings and treatment planning.
  • Demonstrate knowledge of PART techniques.
  • Keep the supervisor informed of client behavior and condition.
  • Notify supervisor of those clients who are clinical management concerns.
  • Participate in the assessment of newly admitted clients.
  • Observe, record, and report client social, medical, and psychiatric behaviors.
  • Demonstrate knowledge and proper use of equipment and supplies.
  • Promote and assist clients in developing self-help skills.
  • Demonstrate good rapport and cooperative working relationships with all members of the team.
  • Continually focus on assigned tasks, seek, and implement improvements.
  • Understand and demonstrate the safety program in all activities.
  • Protect clients from behavior that could damage themselves or others.
  • Demonstrate knowledge of Telecare’s customers including clients, their families, and governmental agencies in all interactions and conduct.
  • Act in a professional manner, always demonstrating respect and understanding of the community and neighborhood when representing Telecare.
  • Understand and demonstrate knowledge of all policies with attendance.
  • Attend all assigned in-service education classes.
  • Role model excellent professional practice.
  • Dress appropriately and professionally.
  • Maintain professional licensure and certificates.
