About The Position

La Casa Mental Health Rehabilitation Center (MHRC) is a 190-bed program providing assistance to adults 18 and older. The organization's purpose is to help people with mental impairments realize their full potential, and its mission is to deliver and manage excellent services and systems of care for persons with serious mental illness. The Licensed Vocational Nurse/Licensed Psychiatric Technician (LVN/LPT) provides services related to the safe and appropriate administration of medical treatment, including medications, as prescribed by the physician. The LVN/LPT is also responsible for documenting the client's response to treatment for the Medical Director and Treatment Team and for notifying all parties of any clinical changes in the client's condition.

Requirements

  • Graduation from an accredited Licensed Vocational Nurse or Licensed Psychiatric Technician program
  • Current license by the State of California
  • A valid CPR Certificate
  • Demonstrated understanding of psychiatric illness
  • Knowledge of Psychiatric Rehabilitation principles
  • Knowledge of psychotropic medications
  • Knowledge of the laws and regulations governing the delivery of mental health services in the State of California
  • Receive clearance from the Department of Justice

Nice To Haves

  • One (1) year of experience in psychiatric nursing in a skilled nursing facility or an acute hospital

Responsibilities

  • Provide safe, effective, and efficient direct care in accordance with established policies, procedures, and standards of care.
  • Establish and maintain a therapeutic relationship with clients.
  • Attend and participate in community meetings and groups.
  • Monitor the activities and documentation of the MHW’s on the assigned wing.
  • Coordinate and participate in counting medications and giving report at the beginning and end of each shift.
  • Coordinate admission of new clients by designating the admitting MHW.
  • Complete admission documentation including the problem list and the I.D. note.
  • Coordinate discharges by ensuring discharge orders are written, and that discharge medication and clothing are given to the client.
  • Complete appropriate discharge documentation.
  • Transcribe physician orders accurately and in a timely manner.
  • Administer medication and treatment to clients.
  • Receive and present change of shift report.
  • Monitor the monthly recap of physician orders, medication record and treatment records.
  • Review all medical and psychological reports.
  • Inform the physician and nursing administrator of any abnormalities found in the client’s condition or laboratory reports.
  • Complete the twenty-four hour Supervisors Report and the Charge Nurse’s Report each shift.
  • Participate as a team member and provide input via reporting observations, concerns and asking appropriate questions.
  • Review and implement client treatment plans.
  • Actively participate in multidisciplinary team meetings and treatment planning meetings.
  • Demonstrate knowledge of Professional Assault Response Training (PART).
  • Keep the supervisor informed of client behavior and condition.
  • Notify the supervisor of those clients who are clinical management concerns.
  • Observe, record and report client social, medical and psychiatric behavior.
  • Demonstrate knowledge and proper use of equipment and supplies.
  • Promote and assist clients in developing self-help skills.
  • Demonstrate a good rapport and cooperative working relationships with all members of the team; respond to co-workers with concern and promote group morale.
  • Continually focus on assigned tasks and seek and implement improvements.
  • Understand and demonstrate the safety program in all activities.
  • Protect clients from behavior that could damage themselves or others.
  • Practice universal precautions at all times.
  • Demonstrate a knowledge of Telecare’s customers including clients/clients, families and governmental agencies in all interactions and conduct.
  • Act in a professional manner, demonstrate respect and understanding of the community and neighborhood when representing Telecare in the community.
  • Complete and follow through with tasks and assignments, meeting expected deadlines.
  • Understand and demonstrate knowledge of all policies associated with attendance.
  • Attend all assigned in-service education classes.
  • Role model excellent professional practice.
  • Always dress appropriately and professionally.
