About The Position

The Lung Health Program Coordinator position is grant-funded by the Connecticut Early Detection & Prevention Program at Hartford HealthCare. The Connecticut Early Detection & Prevention Program (CEDPP) is a comprehensive screening program available throughout Connecticut for medically underserved women. The primary objective of the program is to significantly increase the number of women who receive Lung cancer screening, diagnostic and treatment referral services. Requirements

  • Bachelor’s Degree strongly preferred with three or more years of experience working in community health. High School diploma with 10 years’ experience in community health in lieu of a degree.
  • Self-directed and motivated individual with flexibility and collegiality.
  • Strong organizational, analytical, problem-solving and communication skills in a highly visible and responsible position.
  • Experience with multi-tasking.
  • Proficient in speaking and writing in English and Spanish.
  • Some weekend and evening hours required.
  • Must have valid driver’s license and personal transportation.

Responsibilities

  • Enroll eligible participants into the CT Early Detection and Prevention Program (CEDPP).
  • Assess eligible participants for barriers preventing participation in early detection screening and prevention services and eliminate barriers as possible.
  • Assess each client for or refer to available insurance programs before any program services are rendered.
  • Participate in health education and outreach activities to individuals in eligibility priority populations, to assist in identifying eligible participants for the CEDPP.
  • Develop, identify, enroll, track and conduct navigation to patients who would benefit from coordination of care approach in CEDPP.
  • Identify high-risk patients and manage them with the goal of improving quality of health outcomes.
  • Ensure patients receive testing and referrals for treatment, as needed, in timeframes given by DPH and follow-up with appropriate resources identified.
  • Educate clients regarding the importance of screening and regular screening intervals.
  • Ensure case management services are offered and provided to those clients who have abnormal results, as specified, and who need case management to facilitate care.
  • Document all assessments, patient interactions, and physician interactions, including all education-related encounters.
  • Utilize documentation tools such as EPIC and MED-IT to ensure accurate, concise and consistent documentation.
  • Obtain, coordinate and submit complete and accurate patient data for services rendered by data submission guidelines.
  • Serve as a liaison between HHC and subcontractor(s) to ensure effective and efficient program operations.
  • Work closely with medical providers and clinical personnel to ensure that clients have integrated, comprehensive, coordinated healthcare.
  • Work with subcontractors to submit invoices for payment on a timely basis.
  • Work collaboratively with CEDPP team and provide DPH with monthly and tri-monthly reporting.
