The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the hospice plan of care, providing treatments, and working collaboratively with the interdisciplinary team to support comfort-focused patient care. Requirements
- Completion of an accredited practical (vocational) nursing program
- Currently licensed as a Licensed Practical Nurse or Licensed Vocational Nurse in the state
- Must be a licensed driver with an insured and reliable automobile
- Maintains current CPR certification
- Complies with accepted professional standards and practice
- Demonstrates strong verbal and written communication skills
- Strong organizational skills
Nice To Haves
- Three years nursing experience preferred
- Hospice, home health, or medical-surgical experience preferred
Responsibilities
- Provides direct patient care as defined in the State’s Nurse Practice Act
- Implements plan of care initiated by the registered nurse
- Provides accurate and timely documentation consistent with the plan of care
- Collects assessment data and reports observations to the RN and physician as appropriate
- Assists in coordination of hospice services with interdisciplinary team members
- Provides patient and caregiver education and emotional support
- Uses equipment and supplies effectively and efficiently
- Participates in professional growth and development
- Performs other duties as assigned by the registered nurse
