About The Position

The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the hospice plan of care, providing treatments, and working collaboratively with the interdisciplinary team to support comfort-focused patient care. Requirements

  • Completion of an accredited practical (vocational) nursing program
  • Currently licensed as a Licensed Practical Nurse or Licensed Vocational Nurse in the state
  • Must be a licensed driver with an insured and reliable automobile
  • Maintains current CPR certification
  • Complies with accepted professional standards and practice
  • Demonstrates strong verbal and written communication skills
  • Strong organizational skills

Nice To Haves

  • Three years nursing experience preferred
  • Hospice, home health, or medical-surgical experience preferred

Responsibilities

  • Provides direct patient care as defined in the State’s Nurse Practice Act
  • Implements plan of care initiated by the registered nurse
  • Provides accurate and timely documentation consistent with the plan of care
  • Collects assessment data and reports observations to the RN and physician as appropriate
  • Assists in coordination of hospice services with interdisciplinary team members
  • Provides patient and caregiver education and emotional support
  • Uses equipment and supplies effectively and efficiently
  • Participates in professional growth and development
  • Performs other duties as assigned by the registered nurse
