About The Position

LPN Supervisor for Personal Care Elm Terrace Gardens (CCRC) 12 Hr. Shift - Two (2) Days Per Week - 7:00 pm - 7:00 am & Every Other Weekend - 7:00 pm - 7:00 am We are seeking a dedicated and experienced Licensed Practical Nurse (LPN) Supervisor to lead and oversee nursing staff within our Personal Care Community. This role offers an excellent opportunity to contribute to high-quality patient care while providing leadership and support to our Med Tech’s and PC Aides. The ideal candidate will possess strong clinical skills, leadership abilities, and a commitment to fostering a positive and compliant work environment. We, Elm Terrace Gardens, are a Continuing Care Retirement Community (CCRC) located in the heart of Lansdale, PA and we are looking for compassionate and qualified LPN Supervisor for Personal Care. Role Purpose: To provide professional direct resident nursing care and, in conjunction with the Charge Nurse, to supervise daily nursing activities performed by Nursing Assistants. To promote and maintain a positive physical, social, and psychological environment for residents in the Skilled Nursing Facility. To perform these duties in accordance with applicable current federal, state, and local regulations and standards, and as may be directed by the Charge Nurse; and to perform these duties in accordance with the mission of Elm Terrace Gardens to promote the health and well-being of individual residents based on their functional abilities and potential. Note: Position is subject to a ninety-day orientation period from date of employment. Position is subject to applicable provisions of the federal Fair Labor Standards Act and related regulations. Incumbent becomes eligible for benefits and leave accruals according to the designated employment category after successful completion of the orientation period. Requirements

  • Must be a Licensed Practical Nurse in good standing and be currently licensed by the Commonwealth of Pennsylvania Bureau of Professional and Occupational Affairs, Department of State; or
  • Must be a Licensed Practical Nurse in good standing currently licensed to practice in another state with a temporary permit to practice in the Commonwealth of Pennsylvania, and must obtain Commonwealth licensure within twelve (12) months of employment; or
  • Must be a graduate in good standing of a Commonwealth-approved school of practical nursing with a temporary permit to practice in the Commonwealth after passing the state board examination; and
  • Must be knowledgeable of nursing and medical practices and procedures and laws, regulations, and guidelines governing skilled nursing facilities; and
  • Must be at least eighteen (18) years of age and completed high school or obtained the equivalent of a high school education; and
  • Must possess the ability to speak to and be understood by residents and staff, and demonstrate the ability to read, write, and follow oral and written directions; and
  • Must possess an agreeable and helpful attitude toward senior adults.
  • Has the ability to sit, walk, reach, bend, lift, grasp, push, pull, and has fine hand coordination.
  • Has the ability to read and write in English.
  • Has the ability to communicate with staff, physicians, consultants, residents, families, visitors, and vendors.
  • Has the ability to distinguish smells, tastes, and temperatures.
  • Has the ability to hear and respond to call bells, alarms, and emergency signals.
  • Has the ability to remain calm under stress.
  • Has the ability to follow oral and written directions.
  • Ability to understand and follow training and in-service sessions.

Nice To Haves

  • We prefer that you have 2+ years of Supervisory Experience within a Healthcare Environment, although not required

Responsibilities

  • Supervise and coordinate the daily activities of licensed practical nurses and PC Aides and MED TECHS
  • Ensure compliance with all healthcare regulations, policies, and procedures
  • Provide mentorship, training, and performance evaluations for nursing staff
  • Assist in developing and implementing patient care plans
  • Monitor patient care quality and address any issues or concerns promptly
  • Collaborate with physicians, healthcare teams, and administrative staff to optimize patient outcomes
  • Maintain accurate documentation and ensure confidentiality of patient information
  • Participate in staffing schedules and manage resource allocation effectively
  • Render and administer professional nursing services, including the administration of medications, in accordance with physician's orders and the physician-approved individual resident care plans; and provide direct nursing care as necessary to assigned residents.
  • Admit, transfer, and discharge residents as required; as directed, ensure that rooms are properly. prepared to receive new residents.
  • As directed, notify families or responsible parties about resident expirations or transfers to acute care facilities.
  • Make rounds with residents' physicians as necessary and ensure their completion of record keeping.
  • Consult with physicians in providing care, treatment, diagnostic, and therapeutic services.
  • Notify physicians of incidents and changes in conditions.
  • Accurately complete, maintain, and verify drug inventories, medical forms, physician, and consultant. orders, reports, evaluations, and resident chart entries according to established policies, procedures, and guidelines.
  • Knows and complies with the facility infection control policies and procedures, including the blood- borne pathogen standard, infectious waste disposal, and such other standards as may be promulgated from time to time.
  • Ensure cleanliness and safety of all nursing service and treatment areas.
  • Follow established safety rules and regulations at all times, including fire safety procedures and disaster control.
  • Promptly report accidents, incidents, and unsafe and hazardous equipment or conditions.
  • Present positive endorsement of Elm Terrace Gardens, its mission, programs, policies, procedures, and administration to Elm terrace Gardens residents, staff, visitors, and other constituencies at all times.
  • Report to work as scheduled.
  • Assist with resident transport to and from activities.
  • Assist the Charge Nurse and Staff Nurse as requested in conducting assessments of new residents and reassessments of current residents in order to identify nursing and medical problems and/or needs and to establish goals to address problems or needs.
  • Conduct such assessments according to documentation and time-frame requirements, including the Minimum Data Set and related documents, and according to individual resident care plan development.
  • Observe, document, and promptly report changes in residents' conditions to the Charge Nurse, Director of Nursing, and/or resident physician, including changes in medical condition and levels of function, appetite, alertness, and behavior.
  • Participate as requested in the facility's interdisciplinary care program and quality assurance and related programs.
  • Attend scheduled care conferences, committee meetings, and such other sessions as directed by the Charge Nurse or Director of Nursing.
  • Develop and maintain a rapport with all facility services, residents, physicians, consultants, and families to ensure an effective program of care and quality of life for residents.
  • Maintains resident confidentiality at all times.
  • Treats residents with kindness, dignity, patience, and respect.
  • Knows and complies with the rules of the residents' rights.
  • Adheres to residents' personal property rights at all times.
  • Be observant that residents' medical and psychosocial needs are met.
  • Report to the Charge Nurse and maintain written records of resident complaints and grievances.
  • Follow established policies and procedures for reporting and investigating all resident incidents and allegations of abuse.
  • Recommend to the Charge Nurse the equipment and supply needs for nursing care.
  • Monitor inventory and assist in maintaining adequate supplies and equipment in proper working order.
  • Participate as required in facility surveys and government inspections.
  • Attend and participate in orientation, staff meetings, training, in-service sessions, and other activities as directed by the Director of Nursing.
  • Assist in the orientation and training of new employees as directed by the Charge Nurse.
  • Performs all other duties as assigned.
