Requirements

• Active LPN license in Colorado
• Home health experience preferred
• Strong judgment and decision-making under pressure
• Great communicator and problem solver
• Reliable transportation, valid driver's license, and insurance
• Comfortable working independently in patient homes

Responsibilities

• Be on-call from Saturday 8:00 AM to Monday 8:00 AM
• Triage patient concerns over the phone and determine appropriate care
• Conduct in-person visits when needed
• Collaborate with the care team to resolve urgent home health needs
• Accurately document triage calls and field visits
