About The Position

Macomb Post Acute Care Center is seeking a Licensed Practical Nurse (LPN) to join their team in a full-time, part-time, or PRN role. This position is ideal for nurses who are dedicated to providing consistent, hands-on care and supporting the daily clinical needs of residents in a skilled nursing environment. LPNs in this role are responsible for monitoring resident conditions, administering treatments, and collaborating closely with the broader care team to ensure high-quality outcomes.

Requirements

• Active LPN license in the state of Illinois
• Strong attention to detail and clinical awareness
• Ability to work collaboratively in a team-based environment

Nice To Haves

• Minimum 1 year of nursing experience

Responsibilities

• Provide direct nursing care in accordance with care plans and physician orders
• Administer medications and treatments safely and accurately
• Observe, document, and report changes in resident condition
• Support CNAs and assist with workflow coordination on the unit
• Communicate effectively with nurses, providers, and interdisciplinary team members
