Together, let's drive excellence in healthcare. Working under the direction of supervisor and/or Nurse Manager, directs and takes responsibility for total care of residents on a team. Responsible for implementing interventions designed to achieve the goals and objectives of the resident plan of care. All work is performed within a resident centered framework in collaboration with other healthcare professionals, residents and their families. Functions as a role model for staff. Recognizes the value of each person as an individual. Supports and Promotes Resident/Patient strengths and abilities, capacity for social contribution, unique values, preferences, living habits and autonomy of choice. Embraces a vision of aging that is meaningful and relationship based. Connecting To Mission: All work is performed within a resident centered frame work in collaboration with other healthcare professionals, residents and their families. Requirements

  • Current LPN license
  • Demonstrates position communication skills
  • Ability to relate to staff in a professional manner
  • Must be flexible with scheduling
  • Computer experience preferred
  • Demonstrates sensitivity to residents emotional, social, mental health and spiritual needs
  • Must be able to interact well with physicians and other health care professionals
  • Able to exercise sound professional judgment
  • Able to respond to emergency

Nice To Haves

  • Previous experience preferred
  • Previous experience in a rehab setting preferred.
