This opportunity provides the following: The Lotus Case Manager/Intake Coordinator is responsible for performing a variety of duties within case management, utilization review, and community correspondence in the inpatient setting. These include, but are not limited to, daily contacts with Lotus referral sources and Third-Party payers, discharge planning and aftercare coordination. The Lotus Case Manager/Intake Coordinator will be a liaison between the acute and Lotus adolescent female programs.

Requirements

  • Excellent judgment and clinical skills are essential for this position
  • EDUCATION and/or EXPERIENCE: Graduate from a four year college or university with a degree in a clinical field.
  • An equivalent combination of education and experience may substitute

Responsibilities

  • Managing Lotus calls, referrals and care coordination
  • Works with Lotus patients on any case management items that may come up
  • Responsible for attending off-site activities related to treatment needs
  • Works with clinical team to assist with communication with DCBS and discharge planning
  • Assists in discharge planning preparation as the liaison as patient is finishing programming materials
  • Assists with graduations on the unit and assists with the discharge plan when a patient is graduating or discharging from program
  • Demonstrates the ability to effectively communicate with community partners, clients and staff
  • Demonstrates the ability to prioritize the multiple demands of the position and is able to accurately follow instructions
  • Understands the documentation requirements and completes these requirements in a timely fashion
