About The Position

The Intake Coordinator is responsible for reviewing, assessing, and evaluating case eligibility for CIPP/DCIPP meetings . This role requires strong organizational and communication skills to ensure the timely and efficient coordination of case reviews, scheduling, and documentation . The Intake Coordinator will work closely with internal teams and external partners to facilitate case management processes and improve service delivery for children and families.

Requirements

  • High School Diploma and at least two years of experience in casework, case management, or a related field.
  • Strong organizational, communication, and computer skills.
  • Ability to travel as needed to attend meetings and coordinate services.

Nice To Haves

  • Bachelor’s Degree and two years of professional casework/case management experience
  • Experience in healthcare or Joint Commission-related safety standards is a plus.

Responsibilities

  • Receive case notifications and provide CIPP/DCIPP Universal Referral Forms to Caseworkers for completion.
  • Review and assess completed paperwork to determine case eligibility for CIPP/DCIPP meetings.
  • Research and gather critical case information , including Child Protection reports, medical records, school reports, psychological evaluations, caregiver data, and court documents.
  • Enter referrals into the CIPP/DCIPP scheduling system and distribute referral materials to facilitators.
  • Coordinate and schedule CIPP/DCIPP meetings with identified participants, such as biological parents, children/youth, substitute caregivers, DCFS/POS Supervisors, Caseworkers, and Guardians Ad Litem.
  • Provide caseworkers and supervisors with necessary documentation for CIPP/DCIPP meetings.
  • Build and maintain effective working relationships with internal teams, caseworkers, supervisors, and external partners.
  • Attend and participate in various regional meetings as needed.
  • Assist in quality assurance and improvement efforts related to the CIPP/DCIPP process.
  • Support case triage and emergency review planning with supervisors.
  • Maintain accurate case records within the organization’s electronic system, ensuring compliance with all required guidelines.
  • Generate status and administrative reports for internal teams and leadership.
