Local/Travel Licensed Practical Nurse - LPN 26-00019 Job Title: Local/Travel Licensed Practical Nurse (LPN) Job Location: Falmouth, MA 02540 Duration: 12 weeks (extendable) Shift: 8-hour shifts, 5 days/week, Evenings (3:00 PM – 11:00 PM), every other weekend/holiday Pay Rate: $1,900 – $2,050 (Weekly Gross) Responsibilities: Direct the day-to-day functions of nursing assistants in accordance with current rules, regulations, and guidelines governing long-term care facilities. Meet with assigned nursing staff and support personnel to plan shift services, programs, and activities. Complete and file required recordkeeping forms/charts upon resident admission, transfer, and/or discharge. Encourage attending physicians to review treatment plans and record/sign their orders and progress notes per policy. Receive and document physician telephone orders on the appropriate forms. Transcribe physician orders to resident charts, cardex, medication cards, and treatment/care plans as required. Chart nurses’ notes in an informative and descriptive manner reflecting resident care and responses. Inform nursing personnel of new admissions, arrival times, and room assignments. Participate in facility surveys/inspections by authorized government agencies as requested. Perform nursing administrative functions as outlined in the job description. Participate in orientation of new residents and family members. Make rounds with physicians as necessary. Consult with resident physicians regarding care, treatment, and rehabilitation needs. Review resident charts for treatments, medication orders, and diets. Prepare and administer medications as ordered; verify resident identity prior to administration. Perform all LPN duties related to drug administration. Ensure adequate supplies of medications, equipment, and floor stock; report needs to the Nurse Supervisor. Administer professional services including catheterization, tube feedings, suctioning, dressing changes, colostomy care, specimen collection, and range-of-motion exercises. Collect samples (urine, blood, sputum) and perform routine lab tests. Take and record vital signs (TPR, blood pressure, etc.). Monitor food brought in by family/visitors to ensure compliance with dietary restrictions; report issues. Provide health education to residents and families. Maintain confidentiality of all resident information and comply with resident rights and HIPAA standards. Perform other related duties as assigned. Required Education & Qualifications: Graduate of an accredited LPN program or possess a Nursing Degree from an accredited college or university. Current, active, unencumbered Massachusetts LPN license. BLS/CPR and AED certification required. Ability to read, write, speak, and understand English. Demonstrated knowledge and skills appropriate to age-related resident care needs. Strong teamwork, leadership, and supervisory abilities. Ability to make independent decisions when necessary. Ability to communicate tactfully with staff, residents, families, visitors, and agencies. Knowledge of nursing practices, medical procedures, and applicable regulations. Ability to plan, organize, and implement care programs and policies. Patience, professionalism, enthusiasm, and adaptability. Physical ability to lift at least 50 pounds and meet all job-related physical and sensory requirements. Must not pose a direct threat to workplace safety.
