About The Position

A long-term care facility is seeking an experienced Licensed Practical Nurse (LPN) for a local or travel contract assignment. The LPN will provide direct nursing care, oversee nursing assistants, and ensure high-quality resident care in compliance with state and federal regulations.

Requirements

  • Graduate of an accredited Licensed Practical Nurse (LPN) program
  • Active, unencumbered Massachusetts LPN license
  • BLS/CPR and AED certification (required)
  • Minimum 1 year experience in long-term care or skilled nursing preferred
  • Strong documentation and communication skills
  • Ability to work independently and make sound clinical decisions
  • Leadership and supervisory capability
  • Knowledge of nursing practices, regulations, and long-term care standards
  • Ability to lift at least 50 pounds and meet physical job requirements
  • Ability to read, write, and communicate effectively in English

Responsibilities

  • Direct and supervise day-to-day activities of nursing assistants
  • Plan and coordinate shift-based nursing services and activities
  • Complete admission, transfer, and discharge documentation accurately
  • Receive, transcribe, and implement physician orders
  • Administer medications and treatments per physician orders
  • Monitor residents’ conditions and document nursing notes clearly and timely
  • Communicate new admissions and care updates to nursing staff
  • Participate in facility surveys, audits, and inspections as required
  • Perform nursing administrative and supervisory duties
  • Assist with resident and family orientation to the facility
  • Make rounds with physicians and coordinate resident care plans
  • Review charts for medication, diet, and treatment accuracy
  • Maintain adequate floor stock of medications and supplies
  • Perform clinical procedures including catheter care, tube feedings, wound care, suctioning, and specimen collection
  • Monitor and record vital signs (TPRs, blood pressure, etc.)
  • Provide patient and family education related to health care needs
  • Ensure resident confidentiality and adherence to residents’ rights
  • Support end-of-life care when required
  • Perform additional duties as assigned
