Requirements

  • BSW or college degree in Social Work, plus a minimum of 3 years of experience working with seniors in a healthcare setting
  • Valid PA driver’s license
  • Experience in case management or a related field, including:
  • Assessment and care‑planning skills
  • Knowledge of Medicare/Medicaid reimbursement criteria
  • Familiarity with clinical assessment methods and community resources
  • Strong organizational and file management skills
  • Excellent communication and interpersonal abilities

Responsibilities

  • Maintain professional relationships with Members, physicians, service providers, and all disciplines involved in Member care
  • Continually update clinical assessment skills and knowledge of senior resources
  • Schedule and conduct pre‑admission assessments, including mini‑mental exams, depression screens, nutritional and home assessments, and ADL/IADL level‑of‑care evaluations as needed
  • Manage a caseload of Members through ongoing communication with Members and families, assessments, and coordination of both Plan‑provided and non‑Plan services
  • Initiate and maintain individualized Care Plans, ensuring detailed documentation in Member files and the REPS system
  • Activate care coordination services and oversee care rendered by credentialed vendors
  • Complete accurate, timely monthly billing for assigned caseload
  • Maintain current files on community resources available to seniors
  • Manage the referral database, including identifying new vendors, updating vendor documentation, and distributing referrals
  • Represent SmartLife at professional meetings, networking events, and community organizations
  • Participate in the on‑call rotation
  • Assist the Care Coordination Manager with development and implementation of wellness programs
  • Assist with maintaining and updating the outside referral directory for Members
