About The Position

Under the direct supervision of the supervising RN or Day Center Manager, the PACE Licensed Vocational Nurse (LVN) assists in assessing for and delivering clinical skilled services. This position is based in the Clinic and collaborates with all PACE departments including the Day Center, Home Care and supports PACE activities, including delivering assistance with Participant activities as assigned. Compensation: $31.09 - $32.64 an hour Location: This position will provide coverage for Home Care, Clinical, and Day Center Departments within the Modesto and Merced PACE Centers.

Requirements

  • Valid CA Driver's License, acceptable driving record, and vehicle insurance.

  • Knowledge of safety and infection control requirements for healthcare facilities.
  • Demonstrated experience in quality assurance and performance improvement activities and in the development and implementation of staff education with application of adult learning concepts.
  • Knowledge of nursing best practices, Medi-Cal, Medicare, and community engagement a plus, available community resources.
  • Proficient in Microsoft Office applications.
  • Veni Puncture Certified obtain with-in 1 year of hire.
  • Maintains professional affiliations and any required certifications.
  • High school diploma or equivalent.
  • Graduate of an accredited school of professional nursing.
  • Current unencumbered Licensed Vocational Nurse (LVN) in the state of CA.
  • Current BLS CPR Card certified by American Heart Association.
  • Minimum one (1) year direct patient care experience with skilled nursing or community care experience.
  • Minimum one (1) year experience working with the frail or elderly population

Responsibilities

  • Observe participants for potential medical emergency and provide immediate medical intervention within scope of practice.
  • Observe patients' physical and mental wellbeing along with current medical status and/ or functional capabilities of each participant and report findings to MD or supervising nurse.
  • Provide basic patient education and follow-up as needed to ensure implementation of proposed care plan.
  • Perform basic wound care and check vital signs as directed by attending medical provider.
  • Administration of medication (intramuscular, subcutaneous, inhaled and/ or rectal route) as prescribed by medical provider.
  • Perform venipuncture/IV start as ordered. Process labs and send to diagnostics.
  • Ensure all documentation is complete; document medical history within the system, keep medical records current and upload supporting documentation as needed.
  • Ensure all documentation is completed within a timely manner as per DHCS protocol and assist with department audits as needed.
  • Supervise Patient Care Assistants in accordance with regulations to ensure completion of assigned tasks as identified by Supervising RN.
  • Gather, compile and record all information during Participant encounters in EMR.
  • Assist the Care Coordinator RN as determined by IDT and as needed to ensure compliance.
  • Under the direction of the Supervising RN, assist in completing the assessment tool including diagnoses, medications, ADL's (Activities of Daily Living), IADL's (Instrumental Activities of Daily Living) in addition to a narrative summarizing potential participant's skilled needs.
  • Review medical records for each participant, on-going services and services received within past year using EMR, plan of care for the upcoming year using EMR, and IDT annual assessments to assist in ensuring continuing skilled needs are identified.
  • Attend and participate in staff meetings, in-services, projects, and committees as assigned.
  • Drive to participant's homes as needed.
  • Be flexible in schedule of hours worked.
  • Other projects and duties as assigned.
