About The Position

The Gary and Mary West PACE (GMWP) Clinic LVN is responsible for coordinating and providing direct and indirect nursing care to participants at the center as well as at participant's home, working in close coordination with the PACE Physician, Registered Nurse, clinic staff and other interdisciplinary team (IDT) members to manage ongoing care, complex clinical situations, and to support the operation of the GMWP primary care clinic.

Requirements

  • Graduate of an accredited school of nursing.
  • Unencumbered California Licensed Vocational Nurse (LVN)License.
  • Current Basic Life Support (BLS) / First Aid CPR Certification required.
  • Current driver license.
  • Valid auto insurance.
  • Clean driving record.
  • Minimum of two years nursing experience in a clinical setting with a frail or elderly population.
  • Nursing knowledge and skills necessary to treat frail, elderly participants and manage complex clinical
  • Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency
  • Excellent clinical, organizational and communication skills in settings with seniors, their families and interdisciplinary team members.
  • Experience and competency working with people from diverse backgrounds.
  • Commitment to unlocking the full potential of our most vulnerable seniors.
  • Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

Responsibilities

  • Works under the supervision of the Clinic Manager/Registered Nurse and maintains vocational nursing professional standards of care.
  • Performs home visits to administer prescribed medications and treatments in accordance with nursing standards to supplement clinic services.
  • Observes, records, and reports participant's condition and reaction to medication and treatments to physicians.
  • Documents all interactions related to clinic visits in the electronic medical record.
  • Instructs participants and family members regarding medications and treatment instructions in the home by CNAs, PCWs and homemakers.
  • Keeps Home Care Supervisor and Interdisciplinary team informed on home care issues of participants.
  • Attends morning meetings at center, family meetings, and any other meetings as assigned.
  • Complete all required documentation in a timely and accurate
