As a Home Health Licensed Vocational Nurse (LVN) , you will provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN). This role is ideal for LVNs experienced or interested in home health, geriatric care, dementia care, and community-based nursing .
Requirements
- Active California Licensed Vocational Nurse (LVN) license in good standing
- Graduate of an accredited vocational nursing program
- Minimum 1 year of nursing experience (Home Health experience preferred)
- Valid driver’s license, reliable transportation, and auto insurance
- Strong communication, time management, and documentation skills
- Comfortable using electronic medical records (EMR systems)
Nice To Haves
- Experience with geriatric patients, senior care, or dementia care preferred
Responsibilities
- Provide skilled home health nursing care based on individualized care plans
- Deliver care to geriatric and senior patients in home-based and assisted living settings
- Accurately document visits, treatments, and assessments in the EMR system
- Collaborate with physicians, RNs, families, and interdisciplinary care teams
- Support patients with chronic conditions, recovery, and dementia care needs
- Ensure compliance with California home health regulations and company policies
- Promote patient independence, safety, and overall well-being
