About The Position

Sierra Valley Rehab Center is a skilled nursing facility in Porterville, CA, dedicated to providing compassionate, resident-centered care. We are currently seeking Licensed Vocational Nurses (LVNs) to join our dynamic clinical team. The LVN provides direct nursing care to residents under the supervision of a Registered Nurse or Director of Nursing. This includes administering medications and treatments, supervising Certified Nursing Assistants (CNAs), monitoring patient care, and ensuring adherence to all regulatory and facility standards.

Requirements

  • Current and unencumbered LVN license in the state of California
  • Graduate of an accredited LVN program
  • CPR certification required
  • Prior experience in long-term care or skilled nursing preferred
  • Excellent communication, critical thinking, and leadership skills
  • Proficient in basic computer use and electronic medical records (PCC experience preferred)
  • Ability to lift 25+ lbs and perform frequent standing, walking, bending, and reaching
  • Must be able to respond to emergency situations and provide hands-on care
  • Capable of working under pressure in a fast-paced healthcare environment

Nice To Haves

  • Prior experience in long-term care or skilled nursing preferred
  • Proficient in basic computer use and electronic medical records (PCC experience preferred)

Responsibilities

  • Supervise and support CNAs in delivering quality care
  • Administer medications and treatments as prescribed
  • Chart all care provided, resident responses, and any changes in condition
  • Assist with admissions, discharges, and transfers
  • Participate in care planning, quality assurance initiatives, and staff development
  • Conduct rounds and ensure compliance with nursing procedures
  • Respond to emergencies and provide critical nursing interventions
  • Collaborate with interdisciplinary teams and families
  • Maintain accurate, timely documentation in compliance with state and facility policies
  • Promote resident rights, dignity, and confidentiality at all times
