About The Position

We are seeking an experienced Licensed Practical Nurse (LPN) to provide direct patient care in a Long-Term Care (LTC) setting in Newport, VT. This role involves working collaboratively with Registered Nurses, physicians, and interdisciplinary care teams to deliver safe, high-quality, resident-centered care in accordance with organizational policies, state regulations, and safety standards.

Requirements

• Minimum 2 years of Long-Term Care (LTC) and/or Rehabilitation experience required
• Active Licensed Practical Nurse (LPN) license (state-specific, in good standing)
• Current BLS/CPR certification
• Strong clinical judgment, communication, and documentation skills
• Ability to work full 12-hour day shifts and meet contract requirements

Responsibilities

• Administer medications and prescribed treatments per physician orders
• Monitor, assess, and document resident conditions and responses to care
• Assist residents with activities of daily living (ADLs)
• Collaborate with Registered Nurses, physicians, and interdisciplinary team members
• Identify and promptly report changes in resident condition
• Follow individualized care plans and established clinical protocols
• Maintain compliance with state regulations, safety standards, and organizational policies
