About The Position

Requirements

• Current and valid New York State LPN license.
• Strong clinical assessment and patient care skills.
• Excellent communication and interpersonal abilities.

• BLS certification required.

• Experience in rehabilitation or long-term care nursing preferred.

Responsibilities

• Provide nursing care to residents recovering in a rehabilitation long-term care facility.
• Administer medications and treatments as prescribed.
• Monitor, assess, and document patient conditions accurately.
• Assist residents with activities of daily living and rehabilitation exercises.
• Communicate effectively with patients, families, and healthcare team members.
• Ensure compliance with facility policies and healthcare regulations.
